摘要
本文通过探讨狼疮性肾炎(LN)的中医病因病机及辨治用药规律,为诊治LN提供思路与方法。首先从先天禀赋、外感六淫、日晒毒、药毒及情志失调等方面探讨中医病因病机,并结合诊治LN的临床验案,分析辨治思路及用药特色。笔者认为LN病因复杂,本虚标实为其基本病机,标实以热毒、湿浊、血瘀为主,本虚以肝肾阴虚、气阴两虚、脾肾亏虚为主。运用中医药治疗LN,不仅能够明显改善病情,同时能够减轻西药的不良反应,临床疗效显著。
This article aims to provide ideas and methods for the diagnosis and treatment of lupus nephritis(LN)by exploring the etiology,pathogenesis,and medication rules of traditional Chinese medicine(TCM)in the treatment of LN.Firstly,the etiology and pathogenesis of LN are discussed from the perspectives of innate endowment,external six evils,sun poisoning,drug poisoning,and emotional imbalance.Combined with clinical case studies on the diagnosis and treatment of LN,the thinking and medication characteristics of syndrome differentiation and treatment are analyzed.The author believes that the etiology of LN is complex,with deficiency in essence and excess in superficiality as its basic pathogenesis.The excess in superficiality is mainly manifested as heat-toxicity,damp-turbidity,and blood stasis,while the deficiency in essence is mainly manifested as yin deficiency of liver and kidney,deficiency of qi and yin,and deficiency of spleen and kidney.The use of TCM in the treatment of LN can not only significantly improve the condition,but also reduce the adverse reactions of Western medicine,with remarkable clinical effects.
