摘要
目的了解唇腭裂患者失志综合征现状,并分析疾病不确定感和家庭复原力在唇腭裂患者知觉压力对失志综合征的链式中介作用。方法选取246例唇腭裂患者为研究对象,采用一般资料调查表、疾病不确定感量表(MUIS-A)、中文版家庭复原力评估量表(C-FRAS)、压力知觉量表(CPSS)和中文版失志量表(DS-MV)进行测评;采用Pearson分析疾病不确定感和家庭复原力、知觉压力与失志综合征之间的相关关系;采用Amos软件建立结构方程模型(SEM)并进行链式中介效应检验。结果相关分析结果显示,唇腭裂患者DS-MV与CPSS、MUIS-A均呈正相关(P<0.01),与C-FRAS呈负相关(P<0.01);CPSS与MUIS-A呈正相关(P<0.01),与C-FRAS呈负相关(P<0.01);MUIS-A与C-FRAS呈负相关(P<0.01)。疾病不确定感和家庭复原力在压力知觉与失志综合征之间的链式中介效应成立,总间接效应为45.97%,其中疾病不确定感的特定中介效应占21.87%,家庭复原力的特定中介效应占16.67%,疾病不确定感-家庭复原力的链式中介效应占7.42%。结论疾病不确定感和家庭复原力在压力知觉与失志综合征之间的链式中介作用成立,证实知觉压力可以通过调节疾病不确定感和家庭复原力,间接影响唇腭裂患者失志综合征。
Objective To explore the current status of demoralization syndrome in patients with cleft lip and palate and to analyze the chain mediating role of disease uncertainty and family resilience between perceptual stress and demoralization syndrome in patients with cleft lip and palate.Methods A total of 246 patients with cleft lip and palate were selected and assessed with general information questionnaire,Mishel Uncertainty in Illness Scale for Adult(MUIS-A),Chinese-Family Resilience Assessment Scale(C-FRAS),Chinese Perceived Stress Scale(CPSS)and Demoralization Scale-Mandarin Version(DS-MV).Pearson analysis was used to analyze the correlations among disease uncertainty,family resilience,perceptual stress and demoralization syndrome.Amos software was used to establish structural equation models(SEM)and to test for chain mediating effects.Results Correlation analysis showed that DS-MV was positively correlated with CPSS and MUIS-A(P<0.01),and negatively correlated with C-FRAS(P<0.01);CPSS was positively correlated with MUIS-A(P<0.01),and negatively correlated with C-FRAS(P<0.01).MUIS-A was negatively correlated with C-FRAS(P<0.01).The chain mediating effect of disease uncertainty and family resilience between perceptual stress and demoralization syndrome was established,with a total indirect effect of 45.97%,including 21.87%of the specific mediating effect of disease uncertainty,16.67%of the specific mediating effect of family resilience,and 7.42%of the chain mediating effect of disease uncertainty-family resilience.Conclusion The chain mediating role of disease uncertainty and family resilience between perceptual stress and demoralization syndrome is established,confirming that perceptual stress can indirectly affect demoralization syndrome in patients with cleft lip and palate through modulating disease uncertainty and family resilience.
