摘要
目的:探讨采取左卡尼汀治疗糖尿病合并心力衰竭的效果。方法:选取2022年1月—2023年2月廊坊市人民医院急诊重症医学科收治的糖尿病合并心力衰竭患者82例为观察对象,按照随机数字表法分为观察组与对照组,各41例。对照组给予常规治疗,观察组在常规治疗基础上加用左卡尼汀治疗。比较两组患者临床疗效、糖脂代谢与心功能指标。结果:观察组临床疗效总有效率高于对照组,差异有统计学意义(P=0.008);治疗4周后,两组患者空腹血糖、餐后2 h血糖、甘油三酯与总胆固醇水平均降低,观察组低于对照组,差异有统计学意义(P<0.05);治疗4周后,两组左室射血分数水平均升高,左室舒张末期内径、左室收缩末期内径水平均降低,观察组优于对照组,差异有统计学意义(P<0.05)。结论:采取左卡尼汀治疗糖尿病合并心力衰竭的效果显著,可有效改善患者糖脂代谢及心功能指标。
Objective:To investigate the effect of levocarnitine in the treatment of diabetes mellitus complicated with heart failure.Methods:A total of 82 patients with diabetes mellitus and heart failure admitted to the Department of Emergency Intensive Care Medicine of Langfang People's Hospital from January 2022 to February 2023 were selected as the study subjects.They were divided into observation group and control group according to the principle of random number table,with 41 cases in each group.The control group was given conventional treatment,and the observation group was treated with levocarnitine on the basis of conventional treatment.The clinical efficacy,glucose and lipid metabolism,and cardiac function were compared between two groups.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.008);After 4 weeks of treatment,the fasting blood glucose,2 h postprandial blood glucose,triglyceride and total cholesterol levels in the two groups were decreased,the observation group was lower than the control group,and the difference was statistically significant(P<0.05);After 4 weeks of treatment,the left ventricular ejection fraction level in the two groups were increased,and the left ventricular end-diastolic diameter and left ventricular end-systolic diameter in the two groups were decreased,the observation group was better than the control group,and the difference was statistically significant(P<0.05).Conclusion:Levocarnitine is effective in the treatment of diabetes mellitus and heart failure,and can effectively improve the indexes of glucose and lipid metabolism and cardiac function.
