摘要
目的比较慢性泪囊炎行鼻内镜下泪囊鼻腔吻合术中应用自交联透明质酸钠凝胶与纳吸棉填充的临床价值。方法选取2020年2月至2022年3月收治的48例慢性泪囊炎行鼻内镜下泪囊鼻腔吻合术患者为研究对象,按照随机数字表法将其分为对照组与观察组,各24例。两组患者均行鼻内镜下泪囊鼻腔吻合术,对照组采用纳吸棉填充,观察组采用自交联透明质酸钠凝胶。比较两组的治疗效果。结果术后,观察组的舒适度评分低于对照组,渗血程度优于对照组(P<0.05)。术后,观察组的去甲肾上腺素(NE)、皮质醇(Cor)、肾上腺素(E)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)、基质金属蛋白酶-3(MMP-3)水平低于对照组(P<0.05)。术后,观察组的溢泪程度评分低于对照组,泪膜破裂时间(BUT)长于对照组(P<0.05)。观察组的术后并发症总发生率低于对照组(P<0.05)。结论在慢性泪囊炎行鼻内镜下泪囊鼻腔吻合术患者中,使用自交联透明质酸钠凝胶填充疗效更加明显,且术后渗血量少、创伤小,可减轻疼痛,提高舒适度,减少应激反应与炎性因子分泌,同时可降低术后并发症发生率,值得推广。
Objective To compare the clinical value of self-crosslinked sodium hyaluronate gel and absorbent cotton filling in endoscopic dacryocystorhinostomy for chronic dacryocystitis.Methods Forty-eight patients with chronic dacryocystitis who underwent endoscopic dacryocystorhinostomy from February 2020 to March 2022 were selected as the study objects,and the patients were divided into control group and observation group according to random number table method,with 24 cases in each group.The patients in both groups were given endoscopic dacryocystorhinostomy,the control group was filled with absorbent cotton,and the observation group was filled with self-crosslinked sodium hyaluronate gel.The therapeutic effects of the two groups were compared.Results After operation,the comfort score of the observation group was lower than that of the control group,and the degree of bleeding was better than that of the control group(P<0.05).After operation,the levels of norepinephrine(NE),cortisol(Cor),epinephrine(E),tumor necrosis factor-α(TNF-α),procalcitonin(PCT)and matrix metalloproteinase-3(MMP-3)in the observation group were lower than those in the control group(P<0.05).After operation,the score of tear overflow degree in the observation group was lower than that in the control group,and break-up time(BUT)was longer than that in the control group(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion In patients with chronic dacryocystitis undergoing endoscopic dacryocystorhinostomy,the self-crosslinked sodium hyaluronate gel filling has more obvious curative effect,with less postoperative blood seepage and less trauma,it can alleviate pain and improve comfort level,reduce stress response and inflammatory factor secretion,and reduce the incidence of postoperative complications,which is worth promoting.
