摘要
目的 探究八珍汤、抗生素联合用于老年股骨粗隆间骨折应用股骨近端防旋髓内钉内固定(proximal femoral nail antirotation, PFNA)治疗患者的临床价值。方法 选取2019年1月—2023年1月山东省临沂市兰陵县中医医院骨伤科收治的160例老年股骨粗隆间骨折患者为研究对象,均予以PFNA治疗,采取随机数表法分为对照组、研究组,各80例,分别予以抗生素治疗、抗生素配合八珍汤治疗,对比两组治疗效果。结果 研究组临床总有效率为96.25%高于对照组的87.5%,差异有统计学意义(χ^(2)=4.103,P=0.043)。研究组骨折愈合时间及术后下床时间均短于对照组,差异有统计学意义(P<0.05)。研究组治疗后关节功能评分高于对照组,疼痛评分、白细胞介素-6、肿瘤坏死因子-α、不良事件发生率均低于对照组,差异有统计学意义(P<0.05)。结论 八珍汤+抗生素用于股骨粗隆间骨折(应用PFNA治疗)老年患者,疗效显著。
Objective To investigate the clinical value of the application of proximal femoral anti-rotation intramedullary nail internal fixation(PFNA)treatment in elderly patients with intertrochanteric femur fracture treated with a combination of Bazhen decoction and antibiotics.Methods A total of 160 elderly patients with intertrochanteric fracture of femur treated in the Orthopedics and Traumatology Department of Lanling County Hospital of Traditional Chinese Medicine of Linyi City,Shandong Province from January 2019 to January 2023 were selected as the study objects.PFNA treatment was applied to all patients,and the patients were divided into control group and study group by random number table method,with 80 cases in each group.Antibiotics were given,antibiotics combined with Bazhen decoction were given,respectively.The therapeutic effect of the two groups was compared.Results The total clinical effective rate of the study group(96.25%)was higher than that of the control group(87.5%),the difference was statistically significant(χ^(2)=4.103,P=0.043).The fracture healing time and postoperative getting out of bed time in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The joint function score of the study group after treatment was higher than that of the control group,and the pain score,interleukin-6,tumor necrosis factor-αand the incidence of adverse events were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Bazhen Decoction and antibiotics are effective in elderly patients with intertrochanteric fracture of femur(PFNA treatment).
