摘要
目的 观察兔腰神经根慢性压迫后早期应用药物的治疗效果。方法 纯种新西兰成年兔 2 8只 ,4只作神经定位解剖 ,其于 2 4只全部手术造成腰神经慢性压迫 ,随机分成未处理组和药物组 ,每组又分 10天组、30天组和 180天组 ,每组 4只。压迫模型 :将长 12 mm ,内径 1.5 m m,外径2 .5 mm的硅胶管从椎间外孔缓慢塞入椎间管内 ,压迫腰 6和腰 7左侧神经根直到实验兔被处死 ,右侧为自身对照侧。电生理测定潜伏期 (latent period,L P)和神经传导速度 (nerve conduction ve-locity,NCV)。维生素 B1 2 5 mg,维生素 B1 2 12 5 ug,胞二磷胆碱 6 2 .5 mg,地塞米松 1mg,肌肉注射 ,每日一次 ,共 10日。结果 在未处理组中 ,潜伏期和神经传导速度 10天、30天、180天组的手术侧与非手术侧即左右侧比较均有差异。在药物组中 ,上述指标均无差异。在手术侧的未处理组和药物治疗组天数之间的比较 :未处理组 30天组与 10天组比较 ,潜伏期明显延长 ,神经传导速度减慢 ,而压迫的早期采用药物治疗 ,潜伏期和神经传导速度均未受累。结论 腰神经根慢性受压后 。
Objective To observe the effect of drug on the early period of chronic lumbar nerve root compression Methods Twenty-eight pure New Zealand rabbits were used, of which four were dissected for nerve localization and the rest were randomly classified into untreatment group and drug groups, and each group classfied into 10 -day group ,30- day group and 180 -day group, each group has 4 rabbits Electrophysiological studies were made in all rabbits before killing them on demand Compression model:Silistic tube was filled in intervertebral canal through extra intervertebral foramina to compress the sixth and seventh left lumbar nerve roots Electrophysiological tests included latent period and nerve conduction velocity were analyzed Results of the left side were compared, the right were controlled VitaminB 125mg, VitaminB 12 125mg, Dexamethasone 1mg, Injection Citicolini, 62 5mg, Injectiones intramusculares one time each day, total 10 day Results In untreated group, operation side compared with nonoperation side, the latent period was significantly longer and the nerve conduction velocity was significantly slower in 10 day group?30 day group and 180 day group However, in drug group, the changes of latent period and nerve conduction velocity were not found in all time groups This result illustrated that the nerve root compressed model was successful In operation group, 30day group was compared with 10 day group, the latent period was significantly longer and the nerve conduction velocity was significantly slower If the nerve roots were treated with drug, no change was found in the latent period and the nerve conduction velocity In untreated group at 180day, though the latent period and the nerve conduction velocity of compressed nerve root gradually recovered, compared with drug group, the latent period was significantly prolonged and the nerve conduction velocity was significantly slower Conclusion Using drug earlier on compressed nerve root can relieve edema caused by compression , which has good effect on nerve function recovery
