摘要
目的探究尿白蛋白肌酐比值、尿转铁蛋白以及肾小球滤过率检测在糖尿病肾病患者中诊断价值。方法选取2022年1月—2023年2月麻城市人民医院收治的63例糖尿病肾病患者作为观察组,另选同期入院的43例单纯糖尿病患者作为对照组,分别给予尿白蛋白肌酐比值、尿转铁蛋白以及肾小球滤过率检测,比较两组患者尿白蛋白肌酐比值、转铁蛋白以及肾小球滤过率水平,对比单独检测及联合检测对糖尿病肾病的诊断效能。结果观察组尿白蛋白肌酐比值(57.17±6.73)mg/g、尿转铁蛋白(2.80±0.72)mg/L水平均高于对照组,差异有统计学意义(t=30.079、15.024,P<0.001),肾小球滤过率(47.20±6.93)mL/(min·1.73 m^(2))低于对照组,差异有统计学意义(t=13.265,P<0.001)。受试者工作特征曲线结果显示,单独尿白蛋白肌酐比值、单独尿转铁蛋白、单独肾小球滤过率检测及联合检测诊断糖尿病肾病的敏感度分别为74.48%、73.33%、74.52%、86.20%,特异度分别为82.33%、76.67%、82.66%、91.66%,AUC值分别为0.764、0.779、0.769、0.921,差异有统计学意义(P<0.05)。尿白蛋白肌酐比值、尿转铁蛋白及肾小球滤过率联合检测诊断糖尿病肾病的敏感度、特异度、AUC值均高于单独检测。结论尿白蛋白肌酐比值、转铁蛋白以及肾小球滤过率在糖尿病肾病的诊断方面均具有较高的价值,且联合检测的诊断效能更优,能进一步提高糖尿病肾病诊断的敏感度和特异度,为临床糖尿病肾病患者的诊断和治疗提供参考。
Objective To investigate the diagnostic value of urinary albumin creatinine ratio,urinary transferrin,and glomerular filtration rate testing in patients with diabetic nephropathy.Methods 63 patients with diabetic nephropathy were selected as the observation group from January 2022 to February 2023,and 43 patients with diabetes mellitus alone were selected as the control group,and were given urine albumin creatinine ratio,urine transferrin and glomeru-lar filtration rate tests respectively.The urinary albumin creatinine ratio,transferrin and glomerular filtration rate were compared between the two groups,and the diagnostic efficacy of individual and combined tests for diabetic nephropa-thy was compared.Results The urinary albumin creatinine ratio(57.17±6.73)mg/g and urinary transferrin(2.80±0.72)mg/L levels in the observation group were higher than those in the control group,the differences were statisti-cally significant(t=30.079,15.024,P<0.001),and the glomerular filtration rate(47.20±6.93)mL/(min·1.73 m2)was lower than that in the control group,and the difference was statistically significant(t=13.265,P<0.001).The results of ROC curves showed that the sensitivity of urinary albumin creatinine ratio alone,urinary transferrin alone,glomerular filtration rate assay alone and combined assay for the diagnosis of diabetic nephropathy were 74.48%,73.33%,74.52%,83.20%,the specificity were 74.48%,73.33%,74.52%,83.20%,respectively,the AUC value score ratios were 0.764,0.779,0.769,0.921,and the difference was statistically significant(P<0.05).The sensitivity,specificity,and AUC value of the combined test of urinary albumin creatinine ratio,urinary transferrin,and glomerular filtration rate for the diagnosis of diabetic nephropathy were higher than those of the separate tests,respectively.Conclusion Urinary albumin creatinine ratio,transferrin and glomerular filtration rate have high value in the diagnosis of diabetic nephropathy,and the diagnostic efficacy of the combined test is better,which can further improve the sensitivity and specificity of the diagnosis of diabetic nephropathy,and provide a reference for the diagnosis and treatment of diabetic nephropathy in clinical patients.
