摘要
目的 探讨胫骨髓内钉与胫骨钢板治疗胫骨骨折的临床疗效。方法 选取2019年1月-2022年10月本院收治的80例胫骨骨折患者为研究对象,以数字随机表法随机分成常规组和研究组,每组各40例,其中常规组接受胫骨钢板内固定术治疗,研究组接受胫骨髓内钉固定术治疗,分析治疗效果。结果 研究组手术时间、住院时间、骨折愈合时间均短于常规组,差异具有统计学意义(P<0.05)。研究组生活质量各维度分数均高于常规组,差异具有统计学意义(P<0.05);研究组术后3d、7d及14d的VAS分数均低于常规组,差异具有统计学意义(P<0.05);研究组术后并发症发生率低于常规组,差异具有统计学意义(P<0.05);术后,研究组骨源性碱性磷酸酶指标、Ⅰ型前胶原羧基端肽指标、血清骨钙素指标、超敏C反应蛋白指标及白细胞指标均优于常规组,差异具有统计学意义(P<0.05)。结论 胫骨骨折患者采用胫骨髓内钉治疗的效果更加显著,能有效降低术后的并发症,具有一定安全性,还能够改善实验室相关指标,提升骨代谢情况,缓解疼痛程度,改善患者生活质量,值得推广。
Objective To explore clinical effect of tibial intramedullary nail and tibial plate in treatment of tibial fractures.Methods The report chose 80 patients with tibial fractures admitted to our hospital from January 2019 to October 2022 as data subjects,and randomly divided them into routine group and study group with digital random table method,40 cases in each group,former was treated with tibial plate internal fixation and the latter with tibial bone marrow internal fixation.Treatment effect was analyzed.Results Treatment related indicators:study group had shorter surgical time,hospital stay,and fracture healing time(P<0.05).Life quality:study group had higher scores in all dimensions of life quality,(P<0.05).Pain situation:VAS scores of study group were lower on the 3rd,7th,and 14th days after surgery,P<0.05.Complication situation:Study group had lower incidence of postoperative complications,P<0.05.After operation,bone derived Alkaline phosphatase index,type I procollagen carboxy terminal peptide index,serum osteocalcin index,high-sensitivity C-reactive protein index and leukocyte index in study group were better than routine group(P<0.05).Conclusion Tibial fractures with tibial intramedullary nail can achieve more significant effect,can reduce postoperative complications effectively with certain safety,and improve laboratory related indicators,enhance bone metabolism,alleviate pain,improve life quality of patients,which is worthy of promotion.
