临床分离白色念珠菌耐药性与致病性研究 被引量:1

摘要 目的 通过研究临床分离白色念珠菌(Candida albicans)对药物的敏感性及毒性特点,为探索白色念珠菌致病机制提供线索,对其预防和治疗有重要意义。方法 回顾性分析福建医科大学附属南平第一医院2020年1月—2023年3月临床分离的白色念珠菌125株,均经准确鉴定。采用微量肉汤稀释法进行抗真菌药物的耐药性检测。对比不同菌株和感染部位之间白色念珠菌毒力表型特征的差异。结果 共使用了125株白色念珠菌菌株,对氟康唑、伊曲康唑、伏立康唑、两性霉素B和氟胞嘧啶等抗真菌药物的敏感程度进行了评估。结果显示,除氟胞嘧啶外,其余药物均表现出较高的敏感性,耐药率为1.6%。具体而言,白色念珠菌菌株对氟康唑、伊曲康唑、伏立康唑和两性霉素B均显示出敏感性;不同感染部位的菌株之间毒力因子活性差异不明显。结论 临床分离白色念珠菌对常用抗真菌药物显示出较高的敏感性,因此,临床治疗中对白色念珠菌感染的选药应考虑其敏感性,并且应结合地理位置的因素对毒力进行评估,以制定有效的治疗方案。 Objective To study the drug sensitivity and toxicity characteristics of Candida albicans isolated clinically,provide clues for exploring the pathogenic mechanism of Candida albicans,and have important significance for its prevention and treatment.Methods A total of 125 strains of Candida albicans clinically isolated in Nanping First Hospital Affiliated to Fujian Medical University from January 2020 to March 2023 were included in the study by retrospective analysis,all of which were accurately identified.Antifungal drug resistance was detected by broth dilution method.The differences of virulence phenotypic characteristics of Candida albicans between different strains and infection sites were compared.Results A total of 125 strains of Candida albicans were used to evaluate the sensitivity of fluconazole,itraconazole,voriconazole,amphotericin B,flucytosine and other antifungal drugs.The results showed that all the drugs except flucytosine showed high sensitivity,and the drug resistance rate was 1.6%.Specifically,Candida albicans strains showed sensitivity to fluconazole,itraconazole,voriconazole and amphotericin B.There was no significant difference in virulence factor activity between strains at different infection sites.Conclusion Clinically isolated Candida albicans showed high sensitivity to commonly used antifungal drugs.Therefore,in clinical treatment,drug selection for Candida albicans infection should consider its sensitivity,and the virulence should be evaluated in combination with geographical factors to formulate an effective treatment plan.
