摘要
目的探究在老年股骨近端骨折外周阻滞麻醉中使用右美托咪定复合罗哌卡因的效果。方法选取2018年4月—2021年12月河北省秦皇岛市海港医院收治的305例股骨近端骨折患者,按照随机数表法分为两组,对照组153例采用罗哌卡因行外周神经阻滞麻醉,研究组152例采用右美托咪定复合罗哌卡因麻醉,比对麻醉镇痛效果、患者血流动力、应激反应。结果阻滞后5 min,研究组平均动脉压(mean arterial pressure,MAP)(102.80±7.04)mmHg低于对照组,差异有统计学意义(t=3.571,P<0.05);术后2 d研究组皮质醇(cortisol,COR)、肾上腺素(adrenaline,ADR)水平低于对照组,差异有统计学意义(P<0.05);术后6~24 h研究组疼痛指数、镇静评分低于对照组,差异有统计学意义(P<0.05)。结论右美托咪定复合罗哌卡因复合外周阻滞麻醉效果更佳,老年骨折患者耐受度高,体征更加稳定,应激反应小,兼具麻醉镇痛效果与安全性。
Objective To investigate the effect of using dexmedetomidine compounded with ropivacaine in peripheral block anesthesia for proximal femur fractures in the elderly.Methods 305 patients with proximal femur fractures were selected from April 2018 to December 2021 in Qinhuangdao Haigang Hospital,Hebei Province and were divided into two groups by random number table method.In the control group,153 cases were anesthetized with peripheral nerve block using ropivacaine,and 152 cases in the study group were anesthetized with dexmedetomidine compounded with ropivacaine.The anesthetic analgesic effect,patient hemodynamics,and stress response were compared.Results The mean arterial pressure(MAP)(102.80±7.04)mmHg of the study group was lower than that in the control group at 5 minutes after the block,and the difference was statistically significant(t=3.571,P<0.05).The levels of cortisol(COR)and adrenaline(ADR)in the study group were lower than those in the control group at 2 days postoperatively,and the difference was statistically significant(P<0.05).Pain index and sedation score of the study group were lower than those of the control group 6-24 h postoperative,and the difference was statistically significant(P<0.05).Conclusion Dexmedetomidine compound ropivacaine compound peripheral block anesthesia effect is better,the elderly fracture patients tolerance degree is high,the signs are more stable,the stress reaction is small,both anesthesia and analgesia effect and safety.
