摘要
目的了解深圳市不同血清型登革病毒共感染情况。方法收集2015年—2022年疑似登革热患者血液标本,对核酸阳性者进行型别鉴定,分析不同血清型登革病毒共感染率。同时对复合感染病例采用RT-PCR方法扩增病毒E基因序列并测序,与不同国家和地区的登革毒株进行同源性比较和进化分析。结果2015年—2022年深圳市共报告登革热病例900例,其中810例核酸阳性标本分型结果显示DENV-1占78.15%(633/810),DENV-2占13.70%(111/810),DENV-3占7.04%(57/810),DENV-4占0.99%(8/810);仅发现1例DENV-1和DENV-2复合感染病例。共感染流行株D1/19YS00077/Shenzhen/2019株与1型标准株HAWAII 45株E基因核苷酸和氨基酸同源性分别为94.1%、96.8%,D2/19YS00077/Shenzhen/2019株与2型标准株NGC株E基因核苷酸和氨基酸同源性分别为93.4%、97.6%。进化树显示该流行株最有可能来源于东南亚柬埔寨地区,属于输入病例,与流行病学调查结果一致。结论深圳市登革热流行已从单一血清型向多种血清型并存变迁,同时出现不同血清型共感染现象,此发现有助于深圳市登革热流行病学的研究以及防制工作的开展。
Objective This paper aims to understand the co-infection status of different dengue virus serotypes in Shenzhen during 2015-2022.Methods Blood samples of suspected dengue fever patients from 2015 to 2022 were collected,and the positive nucleic acids were identified by type,and the co-infection rate of different serotypes of dengue virus was analyzed.E gene sequence of the virus was amplified and sequenced by RT-PCR in the cases of complex infection,and homological comparison and evolutionary analysis were carried out on dengue strains in different countries and regions.Results A total of 900 cases of dengue fever were reported in Shenzhen during 2015-2022.The typing results of 810 positive nucleic acid samples showed that DENV-1 accounted for 78.15%(633/810),DENV-2 accounted for 13.70%(111/810),DENV-3 accounted for 7.04%(57/810),DENV-4 accounted for 0.99%(8/810).Only one case of co-infection of DENV-1 and DENV-2 was found.The homology of nucleotide sequence and the deduced amino acid sequence of E gene of D1/19YS00077/Shenzhen/2019 with standard type 1 dengue virus HAWAII45 strain were 94.1%and 96.8%.The homology of nucleotide sequence and the deduced amino acid sequence of E gene of D2/19YS00077/Shenzhen/2019 with standard type 2 dengue virus NGC strain were 93.4%and 97.6%.The evolutionary tree showed that the strain was most likely from Cambodia in Southeast Asia and was an imported case,which was consistent with the epidemiological investigation results.Conclusion The prevalence of dengue fever in Shenzhen has changed from a single serotype to the coexistence of multiple serotypes,and co-infection of different serotypes has occurred at the same time.This finding is helpful to the research of epidemiology and the prevention and control of dengue fever in Shenzhen.
