摘要
目的利用概率回归和截尾回归模型,估算儿童常见恶性肿瘤伤残权重(disability weight,DW),为儿童恶性肿瘤疾病负担评估提供先决条件。方法采用配对比较法和人数健康等值法进行面对面调查,使用Probit回归和截尾回归对数据进行处理,通过线性回归和逆logit转化把DW锚定在0~1之间。结果本调查纳入288名受访者,女性189人(65.6%);年龄<35岁185人(64.2%);本科学历及以上139人(15.2%);具有医学背景67人(23.3%)。9种儿童恶性肿瘤的DW估算值范围在0.136~0.662之间,最大为中枢神经系统肿瘤0.662(95%UI:0.604~0.733),最小为肾母细胞瘤0.136(95%UI:0.074~0.208)。亚组分析发现不同人群对同一种儿童恶性肿瘤DW的评估存在差异。结论儿童常见的恶性肿瘤均对儿童健康造成严重的损伤,最重为中枢神经系统肿瘤,最轻为肾母细胞瘤。
Objective To estimate the disability weight(DW)of common malignancies in children using probability regression and truncated regression models,and to provide a prerequisite for the burden assessment of childhood malignancies.Methods A face-to-face survey was conducted using the paired comparison method and the person trade-off method.Probit regression and truncated regression were used to process the data,and the DW was anchored between 0 and 1 through linear regression and inverse logit transformation.Results A total of 288 participants were included in the survey,of whom 189(65.6%)were female,185(64.2%)were under 35 years old,139(15.2%)had a bachelor′s degree or higher,and 67(23.3%)had a medical background.The estimated DW values of nine types of childhood malignancies ranged from 0.136 to 0.662,with the highest being central nervous system tumors(0.662,95%UI:0.604~0.733)and the lowest being Wilms tumor(0.136,95%UI:0.074~0.208).Subgroup analysis found differences in DW evaluations of the same childhood malignancy among different populations.Conclusion Common childhood malignancies cause serious harm to children′s health,with central nervous system tumors being the most severe and Wilms tumor being the lightest.
