摘要
目的分析2018年宁夏糖尿病患者的灾难性卫生支出及影响因素,为宁夏回族自治区内精准健康扶贫提供依据。方法依据不同的阈值标准对灾难性卫生支出、平均差距、相对差距进行测算,并采用结构方程模型对糖尿病家庭灾难性卫生支出的主要影响因素进行分析。结果对灾难性卫生支出影响最大的为是否有其他慢性病。结构方程模型显示,家中人口数、医疗保险形式、是否有60岁以上老年人、户口性质、文化程度、家庭收入不仅可以直接影响灾难性卫生支出,还能通过家庭医疗支出间接影响灾难性卫生支出。结论慢性病共病是造成灾难性卫生支出最主要的原因,并且家中有老年人的低收入农村家庭易发生灾难性卫生支出;减少医疗卫生支出是降低糖尿病家庭灾难性卫生支出的关键措施。
Objective To analyze the catastrophic health expenditure and influencing factors of diabetes patients in Ningxia in 2018,and to provide the basis for targeted health poverty alleviation in Ningxia.Methods The catastrophic health expenditure,average gap,and relative gap were calculated according to different thresholds,and the main influencing factors of catastrophic health expenditure were analyzed by structural equation model.Results The biggest impact on the catastrophic health expenditure was whether there are other chronic diseases;the population number in the family,the form of medical insurance,whether there were elderly people over 60 years old,registered residence,education level,and family income could not only directly affect the catastrophic health expenditure,but also indirectly affect the catastrophic health expenditure through the family health expenditure.Conclusion The multiplechronic conditions are the main cause of cat astrophic health expenditures,and low-income rural households with elderly people are prone to catastrophic health expenditures;reducing medical and health expenditure is the key measure to reduce the catastrophic health expenditure of diabetes families.
