摘要
目的 比较经颅多普勒超声(TCD)与头部磁共振血管成像(MRA)探查颅内大动脉的血管管腔及血流动力学结果,探讨TCD在临床上诊断的可靠性及应用价值。方法 随机选择缺血性脑血管病患者31例,应用TC—2021TCD仪及VISART 1.5T超导MR装置,分析TCD与头部MRA的结果及两者相符性,了解TCD与头部MRA探查脑血管狭窄性病变的可能性及相关性,分析TCD的临床应用价值。结果 31例患者TCD频谱显像良好,TCD与MRA结果相符性良好。以MRA为标准,统计学结果显示,TCD诊断血管狭窄的敏感性是71.15%,特异性达90.31%,假阳性是9.69%,假阴性是28.85%,符合率是87.40%。结论 TCD对诊断颅内严重狭窄血管具有较高的特异性和敏感性。TCD与MRA相符率高,TCD检查不易与其他疾病相混淆,可作为MRA检查的筛选手段。
OBJECTIVE To observe the image of TCD and MRA and investigate the lumen of intracranial large artery trunks and hemodynam-ics.Then to compare the results of TCD&MRA and assess the clinical reliability & practical value of TCD. METHODS We chose 31 patient with ICVD in random.The 31 patients were examined by TCD and MRA at the same ting, in order to analyse the conformance of the results of them. We investigated the possibility and relativity of TCD and MRA in the diagnosis of ICVD and analyzed the ch'nical value of TCD. RESULTS The images of TCD in the 31 patients are clear and the results of TCD and MRA are coincident. According to MRA, the statistical results showed that the sensitivity of TCD in diagnosis of artery stenosis was 71.15%, specificity was 90.31 % , false positive rate was 9.69 % , false negative rate was 28.85% and the corresponding rate was 87.40% . CONCLUSION TCD is very specific and sensitive in the diagnosis of the severely stenotic vessel.The coincidence of TCD and MRA is high.The examination of TCD in ICVD is not easily confused with other diseases, so it could be used as a screening test for MRA.
