奥美拉唑镁肠溶片联合复方枸橼酸铋钾甲硝唑胶囊治疗幽门螺杆菌感染相关消化性溃疡的疗效观察 被引量:4

Efficacy of Omeprazole Magnesium Enteric-coated Tablets Combined with Compound Bismuth Potassium Citrate and Metronidazole Capsules in the Treatment of Peptic Ulcer Associated with Helicobacter pylori Infection

摘要 目的评价奥美拉唑镁肠溶片联合复方枸橼酸铋钾甲硝唑胶囊用于根治幽门螺杆菌(Hp)感染相关消化性溃疡(PU)的有效性及安全性。方法将2020年1月至2022年7月于厦门市同安区新民街道社区卫生服务中心收治并筛选Hp阳性的A期PU患者(98例)行随机、盲法对照临床研究,分为观察组与对照组各49例,分别采用奥美拉唑镁肠溶片联合复方枸橼酸铋钾甲硝唑胶囊治疗、奥美拉唑镁肠溶片治疗,均治疗2周。比较两组患者的胃肠激素水平、应激反应、免疫功能,同时比较两组的Hp清除率、Hp清除时间和溃疡愈合时间,以及治疗有效率、不良反应率。结果与治疗前相比,治疗2周后两组患者的血清丙二醛(MDA)、脂质过氧化物(LPO)、胃动素(MTL)、胃泌素(GAS)水平均下降,一氧化氮(NO)、降钙素基因相关肽(CGRP)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平均升高,且观察组患者的MDA、LPO、MTL、GAS水平均低于对照组患者,NO、CGRP、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平均高于对照组患者,差异有统计学意义(P<0.05)。治疗2周后,观察组患者的Hp清除率均高于对照组患者,Hp清除时间、溃疡愈合时间均短于对照组患者,差异有统计学意义(P<0.05)。观察组治疗有效率高于对照组(93.9%vs.71.4%,P=0.003),观察组与对照组不良反应率比较差异无统计学意义(12.2%vs.20.4%,P=0.274)。结论Hp感染相关PU患者应用奥美拉唑镁肠溶片联合复方枸橼酸铋钾甲硝唑胶囊治疗的有效性、安全性均较高,主要体现在可减轻患者的应激反应,改善胃肠功能,增强免疫功能以及有效根治Hp。 Objective To evaluate the efficacy and safety of omeprazole magnesium enteric-coated tablets combined with compound bismuth potassium citrate and metronidazole capsules for the radical treatment of Helicobacter pylori(Hp)infection-related peptic ulcer(PU).Methods A randomized,blinded controlled clinical study was conducted on Hp-positive patients(98 cases)with stage A PU admitted and screened at Xiamen Tong'an District Xinmin Street Community Health Service Center from January 2020 to July 2022,which were divided into 49 cases each in the observation group and the control group.The observation group was treated with omeprazole magnesium enteric-coated tablets combined with compound bismuth potassium citrate and metronidazole capsules and the control group was treated with omeprazole magnesium enteric-coated tablets treatment,both of them were treated for 2 weeks.Compare the gastrointestinal hormone levels,stress response,immune function of the two groups,and also compare the Hp clearance rate,Hp clearance time and ulcer healing time,as well as the treatment efficiency and adverse reaction rate of the two groups.Results Compared with the pre-treatment,the serum malondialdehyde(MDA),lipid peroxide(LPO),motilin(MTL)and gastrin(GAS)levels of patients in both groups decreased after 2 weeks of treatment,and the nitric oxide(NO),calcitonin gen-related peptide(CGRP),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)levels increased,and the MDA,LPO,MTL and GAS levels of patients in the observation group were lower than those of patients in the control group,and the NO,CGRP,CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)levels were higher than those of patients in the control group,and the difference was statistically significant(P<0.05).After 2 weeks of treatment,the Hp clearance rates of patients in the observation group were higher than those of patients in the control group,and the Hp clearance time and ulcer healing time were shorter than those of patients in the control group,with statistically significant differences(P<0.05).The treatment efficiency of the observation group was higher than that of the control group(93.9%vs.71.4%,P=0.003),and the difference in the rate of adverse reactions between the observation group and the control group was not statistically significant(12.2%vs.20.4%,P=0.274).Conclusions The effectiveness and safety of Hp infection-related PU patients treated with omeprazole magnesium enteric-coated tablets combined with compound bismuth potassium citrate and metronidazole capsules are high,which is mainly reflected in the fact that it can alleviate the stress reaction of patients,improve gastrointestinal function,enhance the immune function as well as effectively eradicate Hp.
