摘要
腹腔镜胃切除术后消化道重建直接影响病人短期恢复及长期生存质量,是胃癌微创外科领域讨论的热点和难点。无论实施何种胃切除术,术者均不可避免地需要选择合理的消化道重建方式。因此,确保消化道重建的安全、降低吻合口相关短期和长期并发症,对改善病人预后及生存质量具有积极而深远的意义。腹腔镜胃切除术后化道重建方式与传统的开放手术相比,在远端胃切除、近端胃切除和全胃切除术中均有其特有的消化道重建方式。各类重建方式孰优孰劣仍然需要高级别研究证据来证实。在临床实践中,术者须根据病人实际情况,结合术者腹腔镜操作经验,选择安全、有效、合理的消化道重建方式。
Digestive tract reconstruction after laparoscopic gastrectomy directly affects the patients short-term recovery and long-term quality of life,which is a hot and difficult tipic in the field of minimally invasive surgery for gastric cancer.No matter what kind of gastrectomy is performed,it is inevitable for the surgeon to choose a reasonable method for digestive tract reconstruction.Therefore,ensuring the safety of digestive tract reconstruction and reducing short-and long-term complications related to anastomosis are of positive and far-reaching significance for improving the prognosis and quality of life of patients.Compared with traditional open surgery,laparoscopic gastrectomy has its own unique digestive tract reconstruction methods in distal gastrectomy,proximal gastrectomy and total gastrectomy.In clinical practice,the surgeon should choose a safe,effective,and reasonable method of digestive tract reconstruction according to the actual situation of the patient and the laparoscopic operation experience of the surgeon.
