摘要
目的分析关节镜下清理术联合胫骨高位截骨术(high tibial osteotomy,HTO)对中早期内翻型膝关节骨性关节炎(knee osteoarthritis,KOA)的治疗效果。方法选取2020年1月—2022年12月期间常熟市第五人民医院骨科收治的60例中早期内翻型KOA患者为研究对象,随机抽签均分为两组,每组30例。对照组采用关节镜下清理术进行治疗,研究组采用关节镜下清理术联合HTO进行治疗。对比两组治疗效果。结果研究组术后疼痛评分为(2.45±1.28)分,低于对照组(4.76±1.45)分,差异有统计学意义(t=6.542,P<0.05)。研究组治疗优良率为96.67%,高于对照组,差异有统计学意义(P<0.05)。对照组术后不良反应发生率高于研究组,差异有统计学意义(P<0.05)。术前,两组生活质量水平比较,差异无统计学意义(P>0.05);术后,研究组生活质量水平优于对照组,差异有统计学意义(P<0.05)。结论对中早期内翻型KOA患者采用关节镜下清理术联合HTO,有利于提高患者治疗有效率,减少不良反应的出现,提升患者术后生活质量水平。
Objective To analyze the therapeutic effect of arthroscopic cleanup combined with high tibial osteotomy(HTO)for middle to early inversion type knee osteoarthritis(KOA).Methods A total of 60 patients with early varus KOA admitted to the Department of Orthopedics,the Fifth People's Hospital of Changshu from January 2020 to December 2022 were selected as the research objects and randomly divided into two groups,with 30 cases in each group.The control group was treated with arthroscopic debridement,and the study group was treated with arthroscopic debridement combined with HTO.The therapeutic effects of the two groups were compared.Results The postoperative pain score of the study group was(2.45±1.28)points,which was lower than that of the control group(4.76±1.45)points,and the difference was statistically significant(t=6.542,P<0.05).The excellent and good rate of treatment in the study group was 96.67%,which was higher than that in the control group,the difference was statistically significant(P<0.05).The postoperative adverse reaction incidence rate of the control group was higher than that of the study group,and the difference was statistically significant(P<0.05).Before operation,there was no statistically significant difference in quality of life between the two groups(P>0.05);after operation,the quality of life of the study group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The use of arthroscopic debridement combined with HTO in patients with early varus KOA is conducive to improving the treatment efficiency of patients,reducing the occurrence of adverse reactions,and improving the quality of life of patients after surgery.
