摘要
目的了解肿瘤专科医院手术室护士术中压力性损伤评估现状,分析其障碍因素,为临床推进术中压力性损伤风险评估工作提供参考。方法采用质性研究的现象学研究方法,通过目的抽样方法对某肿瘤专科医院手术室14名护士进行半结构化访谈,采用Colaizzi七步分析法分析访谈资料。结果提炼出评估术中压力性损伤风险障碍因素5个主题,包括手术室护士术中压力性损伤风险评估认知程度较低、术中压力性损伤风险评估工具有待完善、评估时机及对象不统一、缺乏多方协作和配合、评估结果责任风险并未界定。结论肿瘤专科医院需重视手术室护士IAPI风险评估知识培训,提高护理人员的知信行;优化肿瘤手术病人IAPI风险评估管理策略;提升多方协作与配合,信息共享、责任共担,保障风险评估工作的准确性及有效性。
Objective To investigate the current situation of intraoperative stress injury assessment(ISIA)by nurses in the operating room of a cancer center and explore the factors impeding the assessment so as to provide references for the clinical promotion of the assessment.Methods The phenomenological research method of qualitative research was used to conduct semi-structured interviews with 14 nurses in the operating room of the cancer center through purposive sampling.The acquired data were analyzed using Colaizzi's seven-step analysis method.Result Five themes related to the factors impeding ISIA were extracted,e.g.,low awareness of ISIA among the nurses,underdeveloped tools for ISIA,un-matching between the interview time and the interviewers,lack of multiparty collaboration and cooperation,and delineation of the responsibility risks of assessment results.Conclusion Cancer hospitals should attach importance to the training of operating theatre nurses on the knowledge of ISIA,improving their ability of knowledgeattitude-belief-practice.Meanwhile,they should also optimize ISIA management strategies for tumor patients undergoing surgeries,enhance coordination from multiple parties and facilitate information sharing and responsibility taking so as to ensure the accuracy and effectiveness of ISIA.
