超声技术对于甲状腺癌、甲状腺腺瘤的诊断与鉴别作用 被引量:1

Analysis of the Role of Ultrasound Technology in the Diagnosis and Differentiation of Thyroid Cancer and Thyroid Adenoma

摘要 目的探讨超声技术对于甲状腺癌、甲状腺腺瘤的诊断与鉴别作用。方法回顾性纳入2021年4月至2022年12月于成武县人民医院确诊的甲状腺癌患者作为甲状腺癌组,取同期入院确诊的甲状腺腺瘤患者作为甲状腺腺瘤组,分析两组的超声图像特征,比较两组的超声诊断准确率、血流参数及血流分布等级。结果甲状腺癌组39例,女31例,平均(41.39±3.22)岁;甲状腺腺瘤组43例,女34例,平均(56.42±3.19)岁。甲状腺癌组超声图像特征主要表现为:边界模糊,形态明显规则,内部有低回声;少数结节中存在钙化点,且多为细小型砂粒状钙化;甲状腺腺瘤组表现为:瘤体肿块较圆形或椭圆,边界光滑,且清晰,有完整包膜,甲状腺未见增大或是局部有增大,内部呈示低回声;发生囊性改变或合并出血时混合回声表现,多数患者边界结节存在晕圈征象,基本无粗大型钙化。两组超声诊断准确率均较高,对比差异无统计学意义(P>0.05)。甲状腺癌组的血流参数收缩期峰值流速和阻力指数均高于甲状腺腺瘤组,差异有统计学意义(P<0.05)。甲状腺癌组血流分级以3级为主,甲状腺腺瘤组则以1级为主,组间等级分布差异有统计学意义(P<0.05)。结论超声技术在甲状腺癌与甲状腺腺瘤疾病的诊断与鉴别中准确率较高。 Objective To explore the role of ultrasound technology in the diagnosis and differentiation of thyroid cancer and thyroid adenoma.Methods Retrospectively included thyroid cancer patients diagnosed in Chengwu County People's Hospital from April 2021 to December 2022 as the thyroid cancer group,and thyroid adenoma patients admitted in the same period as the thyroid adenoma group,analyzed the ultrasound image characteristics of the two groups,and compared the ultrasound diagnostic accuracy,blood flow parameters,and the grade of blood flow distribution of the two groups.Results There were 39 cases in the thyroid cancer group,31 women,with a mean of(41.39±3.22)years old,and 43 cases in the thyroid adenoma group,34 women,with a mean of(56.42±3.19)years old.The ultrasound images of the thyroid cancer group mainly showed the following characteristics:blurred border,obvious regular shape,internal hypoechoic;calcification points existed in a few nodules,and most of them were fine sand-like calcification;the thyroid adenoma group showed the following characteristics:round or oval tumor mass,smooth and clear border,complete envelope,thyroid gland was not enlarged or locally enlarged,and the internal hypoechoic;the occurrence of cystic changes or combined with hemorrhage,mixed echoes,most of the affected patients had a low echo,and the majority of the patients had a mixed echo.In the case of cystic changes or combined hemorrhage,there were mixed echoes,most of the affected border nodules had halo signs,and there were basically no coarse and large calcifications.The accuracy of ultrasound diagnosis was high in both groups,and the difference was not statistically significant(P>0.05).The peak systolic velocity and resistive index of blood flow parameters in the thyroid cancer group were higher than those in the thyroid adenoma group,and the difference was statistically significant(P<0.05).Blood flow grading in the thyroid cancer group was predominantly grade 3,while grade 1 was predominant in the thyroid adenoma group,and the difference in grade distribution between the groups was statistically significant(P<0.05).Conclusions Ultrasound technology has high accuracy in the diagnosis and differentiation of thyroid cancer and thyroid adenoma disease.
