摘要
目的探讨叙事护理在晚期肺癌患者焦虑、抑郁负性情绪护理中的应用效果。方法选取2020年1月—2021年12月收治的120例晚期肺癌患者作为研究对象,按照组间基线资料可比的原则将其分为对照组和观察组,各60例,对照组接受常规护理,观察组接受叙事护理,比较两组患者负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、生命质量[EORTC生命质量测定量表(QLQ-C30)]、睡眠质量高[匹兹堡睡眠质量指数量表(PSQI)]、自我感受负担[自我感受负担量表(SPBS)]、疾病不确定感[疾病不确定感量表(MUIS)]、护理满意度等。结果护理干预前,两组患者SAS、SDS评分比较,差异无统计学意义(P>0.05);护理干预后,观察组SAS评分、SDS评分低于对照组,差异有统计学意义(P<0.05)。护理干预前,两组患者QLQ-C30评分比较,差异无统计学意义(P>0.05);护理干预后,观察组QLQ-C30评分高于对照组,差异有统计学意义(P<0.05)。护理干预前,两组患者PSQI评分比较,差异无统计学意义(P>0.05);护理干预后,观察组PSQI评分低于对照组,差异有统计学意义(P<0.05)。护理干预前,两组患者SPBS评分和MUIS评分比较,差异无统计学意义(P>0.05);护理干预后,观察组SPBS评分和MUIS评分均低于对照组,差异有统计学意义(P<0.05)。护理干预期间,观察组护理满意程度高于对照组,差异有统计学意义(P<0.05)。结论叙事护理应用到晚期肺癌患者中,能使患者保持良好心态,降低疾病不确定感和自我感受负担,进一步提升其睡眠质量和生命质量,并对护患关系有积极作用。
Objective To explore the application effect of narrative nursing in the care of anxiety and depression negative emotions in advanced lung cancer patients.Methods 120 cases of advanced lung cancer patients admitted from January 2020 to December 2021 were selected as the study subjects,and were divided into the control group and the observation group according to the principle of comparable baseline data between the groups,each with 60 cases;the control group received conventional nursing care,and the observation group received narrative nursing care,and the negative emotions of the patients in the two groups were compared with those of the patients in the Self-rating Arrxiety scale(SAS)and the Self-rating Depression Scale(SDS)],quality of life[EORTC Quality of Life Measurement Scale(QLQ-C30)],high quality of sleep[Pittsburgh Sleep Quality Index Scale(PSQI)],self-perceived burden[Self-Perceived Burden Scale(SPBS)],feeling of uncertainty about the disease[Mishel Uncertainty in Illness Scale(MUIS)],and satisfaction with nursing care.Results Before nursing intervention,SAS and SDS scores of patients in the two groups were compared,and the diff erence was not statistically significant(P>0.05);after nursing intervention,SAS scores and SDS scores of the observation group were lower than those of the control group,and the diff erence was statistically significant(P<0.05).Before nursing intervention,the QLQ-C30 scores of the two groups of patients were compared,and the diff erence was not statistically significant(P>0.05);after nursing intervention,the QLQ-C30 scores of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Before nursing intervention,the PSQI scores of the two groups of patients were compared,the diff erence was not statistically significant(P>0.05);after nursing intervention,the PSQI scores of the observation group were lower than those of the control group,the diff erence was statistically significant(P<0.05).Before nursing intervention,there was no statistically significant diff erence between the SPBS score and MUIS score of the two groups(P>0.05);after nursing intervention,the SPBS score and MUIS score of the observation group were lower than those of the control group,the diff erence was statistically significant(P<0.05).During the nursing intervention,the observation group's nursing satisfaction was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Narrative nursing applied to patients with advanced lung cancer can enable patients to maintain a good state of mind,reduce the sense of uncertainty about the disease and the burden of self-perception,further improve their sleep quality and quality of life,and have a positive effect on the nurse-patient relationship.
