摘要
目的:为了缓解人口老龄化带来的医疗与养老之间的问题,利用远程医养协同平台为老年人提供精准的整合性医疗服务,探索依托平台进行医养结合服务的模式。方法:搭建远程医养信息化协同平台,依托全市老龄健康服务体系,以北京老年医院为中心,联合17家区级老年健康与医养结合服务中心,为N家社区卫生服务机构、各类养老服务机构及居家老年人提供医养协同服务,形成“1+17+N”的全市老年健康与医养结合服务体系。结果:构建了三级医养联合体,发挥平台的协同功能,提升了老年健康与医养结合的整体服务水平。结论:远程医养协同平台为构建京津冀老年健康与医养结合服务体系提供了技术支撑,将助力我国老年健康和医养结合服务的发展。
Objective To alleviate the problems between medical care and senior care caused by the aging population,the remote medical care and senior services integration platform are used to provide precision and integrated medical services for the elderly,and to explore integration mode for medical care and senior services relying on the platform.Methods The remote medical care and senior services integration information platform was built based on the Beijing senior health service system,which centered on the Beijing Elderly Hospital,and in conjunction with 17 district-level elderly medical and senior service centers.It provides integrated care of medical and senior services for the elderly from many community health centers,various senior care institutions,and the elderly at home,and a"1+17+N"elderly health service system with integration of medical care and senior services was built for the whole city.Results A three-level medical and senior service union is built,and it plays a synergy function and it improves the overall service level for elderly health and integration of medical care and senior services.Conclusion The remote platform for medical care and senior service integration provides technical support for the construction of the Beijing-Tianjin-Hebei elderly health services system.It will help the development of elderly health and medical and senior services integration in China.
