摘要
新冠后症状的常见症状有疲劳、呼吸急促和认知功能障碍,通常会影响患者日常生活,另外,患者也可能出现胸痛、嗅味觉失灵、肌肉无力和心悸等。世界卫生组织已提出3个号召,即“重视”“研究”和“康复”,呼吁各国政府和卫生当局更多关注新冠后症状及受其影响的COVID-19患者。国医大师刘祖贻认为新冠后症状总病机为余毒未清、余邪未尽。治疗上,遵循扶阳补气、祛除阴邪、益气养阴、调理气机、升清降浊的原则,依次提升五脏六腑气化功能,使脾胃调和,阳气充足,六脉平和,阴阳自和。列举国医大师刘祖贻诊治多汗、咳嗽、失眠、胸闷、月经失调5则新冠后症状医案,为临床治疗提供参考与借鉴:多汗病机为脾肾不足、心肺气虚、卫虚不固,治以参苓白术散加减;咳嗽病机为肺脾气虚、瘀毒内结,治以参芪扶正解毒方加减;失眠病机为肾阴亏虚、脾虚湿阻,治以柴郁补肾安神方加减;胸闷病机为气虚瘀阻,治以芪丹护心饮加减;月经失调病机为脾肾两虚,治以四君子汤合五子衍宗丸加减。
Common symptoms of post COVID-19 condition include fatigue,shortness of breath,and cognitive dysfunction,which usually affect patients'daily life.In addition,patients may also experience chest pain,loss of senses of smell and taste,muscle weakness,palpitations and so on.World Health Organization has put forward three calls,namely“recognition”,“research”and“rehabilitation”,calling on governments and health authorities to pay more attention to post COVID-19 condition and the affected patients.National TCM Practitioner LIU Zuyi believes that the general pathogenesis of post COVID-19 condition is that both the residual toxins and residual pathogenic factors are still uncleared.In terms of treatment,the principles of strengthening yang and replenishing qi,eliminating yin pathogenic factors,invigorating qi and nourishing yin,regulating qi activity,ascending the nutrients and descending the turbid should be followed.As a result,the function of qi transformation of the five zang organs and six fu organs can be improved in turn,so that the functions of the spleen and stomach are regulated,yang qi is sufficient,the six pulses can be peaceful,and yin and yang can be harmonized spontaneously.This paper listed 5 medical records of post COVID-19 condition diagnosed and treated by National TCM Practitioner LIU Zuyi,including hyperhidrosis,cough,insomnia,chest tightness,and menstrual disorder,which provided reference for clinical treatment:the pathogenesis of hyperhidrosis included the spleen and kidney deficiency,qi deficiency of the heart and lung,and insecurity of the deficient Wei-defensive qi,and should be treated with modified Shenling Baizhu Powder;the pathogenesis of cough involved qi deficiency of the lung and spleen,internal stagnation of stasis and toxins,and should be treated with modified Shenqi Fuzheng Jiedu Formula;the pathogenesis of insomnia was kidney yin deficiency and spleen deficiency with dampness,and should be treated with modified Chaiyu Bushen Anshen Formula;the pathogenesis of chest tightness was qi deficiency and stasis obstruction,and should be treated with modified Qidan Huxin Drink;the pathogenesis of menstrual disorder was deficiency of the spleen and kidney,and should be treated with modified Sijunzi Decoction combined with Wuzi Yanzong Pill.
