摘要
目的 分析针灸治疗稳定期慢性阻塞性肺疾病(COPD)的选穴规律。方法 以针灸治疗稳定期COPD为主题对中国知网(CNKI)、PubMed、万方数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(SinoMed)进行文献检索及数据提取,使用SPSS Statistics 26.0及SPSS Modeler 18.0软件进行统计分析。结果 针灸治疗稳定期COPD取穴多集中在腰背和胸腹部;取用频次最高的腧穴为肺俞、定喘、肾俞、足三里、膻中、脾俞;取用频次最高的经脉为足太阳膀胱经和任脉;取用频次最高及个数最多的特定穴分别为背俞穴和交会穴;关联规则支持度最高的穴组是肾俞→肺俞;脾俞→肺俞,聚类分析可分为五类。结论 针灸治疗稳定期COPD注重局部取穴,重用足太阳膀胱经之背俞穴,注重整体调控,主穴为肺俞、肾俞、脾俞,配穴以活血、调畅气机及补虚保健为主。
Objective To analyze the rules of acupuncture in stable chronic obstructive pulmonary disease(COPD).Methods Based on the subject of acupuncture for stable COPD,the articles were retrieved from CNKI,PubMed,Wanfang,VIP and SinoMed data base,using SPSS 26.0 and SPSS Modeler 18.0 software for statistical analysis.Results The acupoints were mostly selected at the waist,back,chest and abdomen.The acupoints of foot Taiyang bladder meridian,especially Beishu acupoints,were selected with the highest frequency.Intersection acupoint was the specific acupoint with the largest number.The commonly used acu⁃points were Feishu(BL13),Dingchuan(EX-B1),Shenshu(BL23),Zusanli(ST36),Danzhong(RN17)and Pishu(BL20).The acupoint group with the highest support of association rules was Shenshu(BL23)→Feishu(BL13);Pishu(BL20)→Feishu(BL13)and the cluster analysis can be divided into five categories.Conclusion Acupuncture for stable COPD pays attention to local acupoints,reuses Beishu acupoints of foot Taiyang bladder meridian,and pays attention to overall regulation.The main acu⁃points are Feishu(BL13),Shenshu(BL23)and Pishu(BL20),and the matching acupoints are mainly regulating Qi mechanism and tonifying deficiency.
