肺部肿瘤快速旋转调强治疗分次间位置不确性分析

Analysis of intersectional position uncertainty in lung tumors with rapid rotation intensification

摘要 目的:采用瓦里安EDGE加速器机载的锥形束CT(CBCT)获取肺部肿瘤快速旋转调强(RapidArc)治疗分次间摆位误差值,探究位置不确定性(IFPU)在空间八个卦限中的分布规律,为移动靶区的精准摆位和治疗提供参考。方法:回顾性选取2019年1月至2022年3月于甘肃省人民医院行RapidArc治疗的肺部肿瘤患者30例,其中男性21例,女性9例,使用热塑体膜仰卧位固定,采用CBCT获取所有患者治疗前共计643次位置信息。结果:IFPU偏向分布卦限次数最大为:10.64±3.95、6.10±2.55和2.87±1.37;百分比最大为(%):45.72±14.61、25.82±8.4和11.97±4.69。结论:肺部肿瘤患者IFPU并非均匀分在8个卦限中,而是集中分布在Ⅱ和Ⅲ卦限中,可能造成治疗计划实际传递剂量整体向该卦限方向偏移。监测肺部肿瘤个体化IFPU的动态变化,预测未来放射治疗阶段可能偏向的卦限并进行适当的干预,需进一步探索。
