摘要
目的探讨临床药师在抗菌药物相关药物热诊疗中的作用。方法回顾性收集医院2014年1月至2021年12月收治的使用抗菌药物后出现药物热的住院患者79例,按临床药师是否干预诊疗过程分为干预组(40例)和对照组(39例)。两组患者在住院期间均接受常规临床诊疗,对于干预组患者,临床药师以药学查房或药学会诊等形式干预其诊疗过程。结果2016年至2021年,临床药师对该病的干预率由22.22%提升至85.00%。干预组药品不良反应(ADR)再发生率、住院抗菌药物使用强度、抗菌药物升级治疗率、特殊使用级抗菌药物使用率及住院抗菌药物静脉输液占比均明显低于对照组,抗菌药物降阶治疗率明显高于对照组(P<0.05)。干预组患者发热至停用疑似药时间及对症处理费用与对照组相当(P>0.05)。结论临床药师参与抗菌药物相关药物热诊疗,可优化临床抗菌药物的应用及保障治疗方案合理。
Objective To investigate the role of clinical pharmacists in the diagnosis and treatment of antibiotics-related drug fever.Methods A total of 79 inpatients with antibiotics-related drug fever admitted to the hospital from January 2014 to December 2021 were collected retrospectively and divided into the intervention group(40 cases)and the control group(39 cases)according to whether clinical pharmacists participating in the diagnosis and treatment of disease.Both groups received the routine clinical diagnosis and treatment during hospitalization,on this basis,the intervention group received intervention in the forms of pharmaceutical ward round or pharmaceutical consultation from clinical pharmacists.Results From 2016 to 2021,the intervention rate of clinical pharmacists for this disease increased from 22.22%to 85.00%.The recurrence rate of adverse drug reaction(ADR),antibiotics use density,use rate of high-grade antibiotics,use rate of antibiotics for special use and proportion of cases receiving intravenous infusion of antibiotics in the intervention group were significantly lower than those in the control group,and the use rate of low-grade antibiotics in the intervention group was significantly higher than that in the control group(P<0.05).The time from the onset of drug fever to withdrawal of suspected drugs and the costs of symptomatic treatment in the intervention group were similar to those in the control group(P>0.05).Conclusion The clinical pharmacists'practice in the diagnosis and treatment of antibiotics-related drug fever can optimize the application of clinical antibiotics and ensure the rationality of treatment plans.
