摘要
目的:了解山东地区儿童肺炎支原体(MP)肺炎的流行特征,并分析12岁以下儿童MP感染抗体阳性患儿的血常规特点,为临床预防及早期鉴别诊断提供参考依据。方法:收集2017年1月-2019年12月笔者所在医院6827例呼吸道感染患儿临床资料。分别检测其肺炎支原体抗体和血常规。结果:6827例患儿中MP抗体阳性率为20.73%(1415/6827),2017-2019年MP抗体阳性率逐年升高(14.72%、22.33%、24.13%),差异有统计学意义(P<0.001)。冬季阳性率最高(22.81%),秋季次之(20.75%),夏、春季节偏低(17.41%、18.31%),不同季节阳性率差异有统计学意义(P<0.001)。女性患儿阳性率高于男性(22.92%,18.84%),差异有统计学意义(P<0.001)。7~12岁阳性率最高(33.56%),4~6岁次之(23.94%),<1岁、1~3岁、13~15岁较低(4.57%、12.14%、16.48%),不同年龄段差异有统计学意义(P<0.01)。1024例不同年龄段MP阳性患儿血常规检测,白细胞计数、淋巴细胞升高率、单核细胞升高率随年龄降低,中性粒细胞升高率随年龄升高,差异有统计学意义(P<0.05)。结论:2017-2019年本地区儿童肺炎支原体感染率逐年升高,秋冬季高发,女患儿高于男患儿,且7~12岁儿童更为常见。
Objective:To understand the epidemiological characteristics of Mycoplasma pneumoniae(MP)pneumonia in children of Shandong Province,and to analyze the blood routine characteristics of children under 12 years old with MP antibody-positive pneumonia infection,so as to provide reference for clinical prevention and early differential diagnosis.Methods:The clinical data of 6827 children with respiratory tract infections from January 2017 to December 2019 were collected.Their Mp antibodies and blood routine tests were performed.Results:The MP antibody positivity rate among 6827 children was 20.73%(1415/6827),and the MP antibody positivity rate increased year by year from 2017-2019(14.72%,22.33%,24.13%),with a statistically significant difference(P<0.001).The positive rate was highest in winter(22.81%),followed by autumn(20.75%)and lower in summer and spring(17.41%and 18.31%),(P<0.001).The positive rate was higher in female children than in male(22.92%,18.84%),and the difference was statistically significant(P<0.001).The positive rate was highest in 7-12 years old(33.56%),followed by 4-6 years old(23.94%),and lower in<1 year old,1-3 years old,and 13-15 years old(4.57%,12.14%,and 16.48%),with statistically significant differences in different age groups(P<0.01).1024 children with positive MP in different age groups had routine blood tests,and the rate of elevated white blood cell count,lymphocytes,monocyte elevation rate decreased with age,and neutrophil elevation rate increased with age,and the differences were statistically significant(P<0.05).Conclusion:The rate of Mp infection in children increased year by year from 2017 to 2019,with a high incidence in autumn and winter.The incidence of Mp was higher in female children than in male children,and it was more common in children aged 7 to 12 years.
