摘要
目的:对比全腹膜外疝修补术中两种疝囊处理方法的应用效果。方法:选择2019年11月至2021年10月收治的94例腹股沟疝患者,按随机数字表法将其分为剥离组(完全剥离疝囊)与横断组(横断疝囊),每组47例,两组均行全腹膜外疝修补术。对比分析两组围手术期指标、疼痛程度、生活质量及并发症发生情况。结果:横断组术中出血量少于剥离组[(12.32±4.11)mL vs.(15.98±4.08)mL],手术时间[(54.13±14.76)min vs.(66.39±18.52)min]、下床活动时间[(1.45±0.50)d vs.(1.97±0.63)d]、住院时间[(5.71±1.24)d vs.(6.37±1.42)d]短于剥离组,术后24 h疼痛轻于剥离组,差异均有统计学意义(P<0.05);两组术后健康调查简表(SF-36)评分及并发症总发生率差异无统计学意义(P>0.05)。结论:相较完全剥离疝囊,全腹膜外疝修补术中横断疝囊具有术中出血少、手术时间短、术后疼痛轻的优势,患者可早期下床活动,是全腹膜外疝修补术中处理疝囊较为有效的方法。
Objective:To compare the effectiveness of two methods of hernia sac management in laparoscopic totally extrape-ritoneal prosthesis(TEP).Methods:Ninety-four patients with inguinal hernia from Nov.2019 to Oct.2021 were selected and divided into the stripping group(47 cases,complete stripping of the hernia sac)and the transection group(47 cases,transection of the hernia sac)according to the random number table method,and TEP was used in both groups.The perioperative indexes,pain level,quality of life and complications in the two groups were compared and analyzed.Results:The amount of intraoperative blood loss[(12.32±4.11)mL vs.(15.98±4.08)mL]in the transection group was less than that in the stripping group,and the operative time[(54.13±14.76)min vs.(66.39±18.52)min],the ambulation time[(1.45±0.50)d vs.(1.97±0.63)d]and the hospital stay[(5.71±1.24)d vs.(6.37±1.42)d]were shorter than those in the stripping group,the pain degree of the transection group was less than that of the stri-pping group 24 h after operation,and the differences were statistically significant(P<0.05).The differences in the postoperative health survey(SF-36)scores and the overall complication rate between the two groups were not statistically significant(P>0.05).Conclusions:Compared with complete dissection of the hernia sac,transverse dissection of the hernia sac during TEP has the advantages of less intraoperative blood loss,shorter operation time and milder postoperative pain,and patients can get out of bed early,it is an effective method for managing the hernia sac during TEP.
