实时剪切波弹性成像及超声造影定量参数与乳腺结节微波消融后疗效的关系 被引量:10

Relationship between quantitative parameters of real-time shear wave elastography and contrast-enhanced ultrasound and efficacy of microwave ablation of breast nodules

摘要 目的 探究实时剪切波弹性成像(SWE)及超声造影(CEUS)定量参数与乳腺结节微波消融后疗效的关系。方法 回顾性选取2019年1月至2022年5月在武汉科技大学附属汉阳医院行微波消融治疗的68例乳腺结节患者为研究对象,所有患者均行SWE及超声造影检查,探究二者对良恶性结节的诊断价值。比较消融前后影像学参数变化情况,根据治疗效果将患者分为观察组(结节体积缩小≥50%,n=53)和对照组(结节体积缩小<50%,n=15)。比较两组临床资料及超声影响特征,采用多因素Logistic回归分析方法分析影响乳腺结节微波消融效果的相关因素。结果 恶性组Emax、Emean、Esd、Eratio、PI值均显著高于良性组,TTP值显著低于良性组,差异均有统计学意义(P<0.05)。消融后,患者乳腺结节Emax、Emean、Esd、Eratio、TTP值显著高于消融前,PI值显著低于消融前,差异均有统计学意义(P<0.05)。观察组乳腺结节消融后Emax、Emean、Esd、Eratio、TTP值均显著高于对照组,钙化占比、PI显著低于对照组,差异均有统计学意义(P<0.05),两组年龄、体重指数、病理类型、结节直径比较,差异均无统计学意义(P>0.05)。多因素Logistic回归分析显示,钙化情况、Emax、Emean、Esd、Eratio、TTP,PI值是乳腺结节患者微波消融疗效的影响因素,其中钙化占比、PI值是影响微波消融疗效的危险因素(P<0.05)。结论 SWE及超声造影可用于乳腺结节微波消融术前诊断及术后疗效评价,其定量参数Emax、Emean、Esd、Eratio、TTP、PI值与消融效果具有显著相关性,可作为消融疗效、术后复发及残余病灶检测的辅助参考指标。 Objective To explore the relationship between quantitative parameters of real-time shear wave elastography(SWE)and contrast-enhanced ultrasound(CEUS)and efficacy of microwave ablation of breast nodules.Methods A total of 68 patients with breast nodules who underwent microwave ablation in the Hanyang Hospital Affiliated to Wuhan University of Science and Technology from January 2019 to May 2022 were retrospectively selected as research subjects.All patients underwent SWE and CEUS,and the diagnostic value of the two on benign and malignant nodules was explored,and the changes in imaging parameters before and after ablation were compared.According to the treatment effect,the patients were divided into observation group(nodule volume reduction of 50%or more,n=53)and control group(nodule volume reduction of less than 50%,n=15).Clinical data and ultrasonic influence characteristics were compared between the two groups,and multivariate Logistic regression analysis was used to analyze the related factors affecting the microwave ablation effect of breast nodules.Results The Emax,Emean,Esd,Eratio and PI in malignant group were significantly higher than those in benign group,while the TTP was significantly lower than that in benign group,the differences were statistically significant(P<0.05).After ablation,the Emax,Emean,Esd,Eratio and TTP of breast nodules were significantly higher than those before ablation while PI was significantly lower than that before ablation,the differences were statistically significant(P<0.05).The Emax,Emean,Esd,Eratio and TTP of breast nodules after ablation in observation group were significantly higher than those in control group,while the proportion of calcification and PI were significantly lower than those in control group,the differences were statistically significant(P<0.05).There were no statistically significant differences in age,body mass index,pathological type and nodule diameter between the two groups(P>0.05).Multivariate Logistic regression analysis showed that calcification,Emax,Emean,Esd,Eratio,TTP and PI were influencing factors for microwave ablation efficacy of patients with breast nodules,and the calcification proportion and PI were risk factors for microwave ablation efficacy(P<0.05).Conclusion SWE and CEUS can be used for preoperative diagnosis and postoperative efficacy evaluation of microwave ablation of breast nodules,and the quantitative parameters Emax,Emean,Esd,Eratio,TTP and PI are significantly correlated with ablation effect,and can be used as auxiliary reference indicators for ablation efficacy,postoperative recurrence and residual lesion detection.
