摘要
目的探讨在治疗胃黏膜病变患者中选择不同牵引方法内镜黏膜下剥离术的价值。方法选取2020年9月—2021年9月在安宁市第一人民医院接受治疗的胃黏膜病变患者80例为研究对象,以随机数表法分成观察组与对照组,每组40例。对照组患者实施牙线牵引辅助内镜黏膜下剥离术治疗,观察组患者实施金属夹弹力圈联合牵引辅助内镜黏膜下剥离术治疗,比较两组治疗效果。结果全体患者手术都取得成功,手术中一次性切除全部病变组织。两组的术中出血量、充分暴露黏膜用时、标记全部病灶用时、切除较难部位用时、切除简单部位用时、手术耗时、住院时间指标比较,差异无统计学意义(P>0.05)。观察组的并发症率为5.00%,对照组并发症率为7.50%,差异无统计学意义(χ^(2)=0.000,P>0.05)。随访两组患者,复发率和转移率比较,差异无统计学意义(P>0.05)。结论在治疗胃黏膜病变患者时,牙线牵引法与金属夹弹力圈联合牵引法都有一定治疗效果,临床需要结合具体情况应用合适的牵引方法,以提升治疗效果。
Objective To explore the value of endoscopic submucosal dissection with different traction methods in the treatment of gastric mucosal lesions.Methods Eighty patients with gastric mucosal lesions who were treated in Anning First People's Hospital from September 2020 to September 2021 were selected as the research objects,and were divided into an observation group and a control group using a random number table method,with 40 cases in each group.The control group was treated with dental floss traction assisted endoscopic submucosal dissection,and the observation group was treated with metal clip elastic ring combined with traction assisted endoscopic submucosal dissection.The therapeutic effect of the two groups was compared.Results The operation was successful in all patients,and all the diseased tissues were removed at one time.The two groups were compared in terms of intraoperative blood loss,time for fully exposing mucosa,time for labeling all lesions,time for resection of difficult parts,time for resection of simple parts,time spent in surgery,length of stay and other indicators,and the situation was basically similar,and the difference was not statistically significant(P>0.05).The complication rate was 5.00%in the observation group and 7.50%in the control group,and the difference was statistically significant(χ^(2)=0.000,P>0.05).Patients in the two groups were followed up,comparison of recurrence and metastasis rates,and the difference was not statistically significant(P>0.05).Conclusion In the treatment of patients with gastric mucosal lesions,both dental floss traction and metal clip elastic ring combined with traction have certain therapeutic effects.It is necessary to apply appropriate traction methods in combination with specific conditions in clinical practice to improve the therapeutic effect.
