摘要
目的:探讨早期与延迟关节镜下前交叉韧带(ACL)重建术在ACL损伤患者中的应用效果。方法:选取2015年1月—2022年12月贵阳市第一人民医院收治的48例ACL损伤患者作为研究对象,根据受伤至关节镜下ACL重建术实施时间分为早期组(受伤3周内)与延迟组(受伤6周后),各24例。两组均行关节镜下ACL重建术。对比两组膝关节功能、膝关节活动度、平衡能力、日常生活能力、生活质量及并发症发生情况。结果:术后6个月,早期组疼痛、功能、活动度、肌力、屈曲畸形、稳定性评分高于延迟组,差异有统计学意义(P<0.001)。早期组膝关节活动度及平衡能力、日常生活能力评分高于延迟组,差异有统计学意义(P<0.001)。早期组生理、心理、社会、环境评分高于延迟组,差异有统计学意义(P<0.001)。早期组并发症发生率低于延迟组,差异有统计学意义(P=0.027)。结论:早期关节镜下ACL重建术治疗ACL损伤的效果更优,能促进膝关节功能恢复,提高患者平衡能力与日常生活能力,减少并发症的发生,改善患者生活质量。
Objective:To investigate the application effect of early and delayed anterior cruciate ligament(ACL)reconstruction under arthroscopy in patients with ACL injury.Methods:A total of 48 patients with ACL injury admitted to the First People's Hospital of Guiyang from January 2015 to December 2022 were selected as the study subjects.According to the time from injury to the implementation of ACL reconstruction under arthroscopy,they were divided into the early group(within 3 weeks of injury)and the delayed group(after 6 weeks of injury),with 24 patients in each group.All patients in two groups received ACL reconstruction under arthroscopy.The knee joint function,knee joint range of motion,balance ability,daily living ability,quality of life,and incidence of complications were compared between the two groups.Results:At 6 months after operation,the pain,function,range of motion,muscle strength,flexion deformity,and stability scores in the early group were higher than those in the delayed group,and the difference was statistically significant(P<0.001).The knee joint range of motion,balance ability,and daily living ability scores in the early group were higher than those in the delayed group,and the difference was statistically significant(P<0.001).The physiological,psychological,social,and environmental scores in the early group were higher than those in the delayed group,and the difference was statistically significant(P<0.001).The complication rate in the early group was lower than that in the delayed group,and the difference was statistically significant(P=0.027).Conclusion:Early ACL reconstruction under arthroscopy in treatment of ACL injury has a better effect,it can promote the recovery of knee joint function,improve balance ability and daily living ability,reduce complications and improve the quality of life of patients.
