摘要
李杲的学术思想深刻影响后世医家对疾病的诊疗,其中阴火理论便是其主要思想之一。干眼可归属于中医学“神水将枯症”“白涩症”“干涩昏花症”等范畴,现临床上多采用滋阴清热,养阴明目法为主。本文主要目的是从阴火理论入手结合“目病多火”的理论,探讨阴火理论与干眼的相互联系。阴火的本质为火,脾胃虚弱,导致的上下阴阳失调是阴火形成的主要病机。笔者认为阴火理论与干眼的关联在于脾胃虚弱,目窍失养。两者联系表现为以下3个方面:肺金受邪,气分伏隐;阴火亢盛,郁蒸目窍;闭塞于下,阴火上攻,治疗上当以脾胃为主兼顾清热。阴火理论为中医在干眼的认识过程中增添新的观点,完善了干眼的中医治疗。
LI Gao´s academic ideas had a profound impact on the diagnosis and treatment of diseases by later generations of doctors,with the theory of Yin Fire being one of the main ideas.The main purpose of this article is to explore the interrelationship between the theory of Yin Fire and dry eye.The essence of Yin Fire is fire and the main pathogenesis of Yin Fire formation is the imbalance of Yin and Yang in the upper and lower parts caused by spleen and stomach weakness.Dry eye can be classified into the categories of"dry eye syndrome","dry astringent eye"and"dry and dim-sighted syndrome"in Traditional Chinese Medicine(TCM).Currently,in clinical practice,the methods of nourishing Yin and clearing heat,nourishing Yin and brightening the eyes are mainly used.Starting from the theory of Yin Fire and combining it with the theory of"eye diseases often correlate with fire",The author believes that the key to the Yin Fire Theory and dry eyes lies in the weakness of spleen and stomach,and the loss of nourishment.The relationship is manifested in the following three aspects:Lung is affected by pathogenic factors,and Qi is divided and concealed;Yin Fire is excessive,causing stagnation in the eyes and orifices;occlusion in the lower part,with Yin Fire attacking upwards,and the treatment mainly focuses on spleen and stomach while also clearing heat.The aim of this article is to add new perspectivesdry eye in TCM,and improve TCM treatment of dry eye.
