摘要
目的探讨司美格鲁肽联合胰岛素对糖尿病患者疗效及对心血管的影响。方法选取2022年4—12月兴义市人民医院内分泌科收治的60例糖尿病患者为研究对象,按随机数表法随分成两组。对照组30例采用甘精胰岛素治疗,治疗组30例给予司美格鲁肽联合甘精胰岛素治疗。治疗20周后比较两组患者的不良反应,以及治疗前后两组胰岛功能指标、血糖及血脂水平。结果两组患者恶心、低血糖、腹泻、急性胰腺炎等不良反应发生率与对照组比较,差异无统计学意义(P>0.05)。治疗后,两组患者的空腹C肽(FCP)、空腹血糖(FPG)、餐后2 h血糖(2 hPG)及糖化血红蛋白(HbA1c)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)、总胆固醇(TC)、胰岛素抵抗指数(HOMA-IR)均有所下降,其中治疗组低于对照组,差异有统计学意义(P<0.05)。治疗后两组血清高密度脂蛋白胆固醇(HDL-C)、胰岛素β细胞功能指数(HOMA-β)升高,其中治疗组高于对照组,差异有统计学意义(P<0.05)。结论司美格鲁肽联合胰岛素能有效降低患者血糖、血脂,改善胰岛细胞功能,维持血糖长期达标。
Objective To investigate the efficacy of semaglutide combined with insulin on diabetic patients and its cardiovascular effects.Methods Sixty diabetic patients admitted to the Department of Endocrinology,Xingyi People's Hospital from April to December 2022 were selected as the study subjects for the study and randomly divided into two groups according to the random number table method.30 cases were treated with glargine insulin as the control group,and 30 cases in the treatment group were given semaglutide combined with glargine insulin.After 20 weeks of treatment,the adverse reactions of patients in both groups were compared,as well as the indexes of pancreatic function,blood glucose and blood lipid levels in both groups before and after treatment.Results There was no statistically significant different in incidence of adverse reactions such as nausea,hypoglycemia,diarrhea and acute pancreatitis in the two groups(P>0.05).After treatment,fasting C-peptide(FCP),fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2 hPG)and glycated hemoglobin(HbA1c),low-density lipoprotein cholesterol(LDL-C),triacylglycerol(TG),total cholesterol(TC)and insulin resistance index(HOMA-IR)decreased in both groups,with the treatment group having lower levels than the control group after treatment,the difference was statistically significant(P<0.05).Serum high-density lipoprotein cholesterol(HDL-C)and insulin beta-cell function index(HOMA-β)increased in both groups after treatment,with the treatment group being higher than the control group,the difference was statistically significant(P<0.05).Conclusion Semaglutide combined with insulin can effectively reduce patients'blood glucose and blood lipids,improve pancreatic islet cell function and maintain long-term blood glucose standard.
