摘要
目的探讨血脂辅助血糖指标检测用于2型糖尿病诊断治疗的临床价值。方法随机选择2021年1月—2022年12月白城中心医院期间收治的50例2型糖尿病患者(观察组)和同期50名体检的健康者(对照组)为研究对象,两组均进行血糖和血脂检测,比较两组检测结果,以糖化血红蛋白≥6.5%且血脂指标中两项或以上指标异常作为诊断试验1阳性的判定标准,以空腹血糖和2 hPG超过正常值上限且血脂指标中两项或以上指标异常作为诊断试验2阳性的判定标准,计算并比较两种诊断试验结果和诊断价值。结果观察组空腹血糖、餐后2 h血糖和糖化血红蛋白水平均较对照组高,差异有统计学意义(P<0.05);观察组总胆固醇、三酰甘油和低密度脂蛋白胆固醇水平均较对照组高,且高密度脂蛋白胆固醇水平较对照组低,差异有统计学意义(P<0.05)。诊断试验1的敏感度、特异度、粗一致性、阳性预测值和阴性预测值均高于诊断试验2,差异有统计学意义(P<0.05)。结论与健康体检者比较,2型糖尿病患者的血糖和血脂检验结果差异均有显著性,有利于指导临床治疗活动;诊断试验1的敏感度、特异度、粗一致性和预测值均处于较高水平且高于诊断试验2,试验1的临床诊断价值较高。
Objective To explore the clinical value of blood lipid-assisted glucose index testing for the diagnosis and treatment of type 2 diabetes mellitus.Methods Fifty patients with type 2 diabetes admitted to Baicheng Central Hospital between January 2021 and December 2022(observation group)and 50 healthy individuals with physical examination during the same period(control group)were randomly selected as study subjects.Blood glucose and lipid tests were performed in both groups,compared the test results of the two groups with glycosylated hemoglobin≥6.5%and abnormalities in two or more of the lipid indexes as the criteria for determining a positive diagnostic test 1,and fasting plasma glucose and 2 hPG exceeding the upper limit of normal value and two or more abnormal lipid indexes were used as the criteria for positive diagnostic test 2,calculate and compare the results and diagnostic value of two diagnostic tests.Results Fasting plasma glucose,2-hour postprandial plasma glucose and glycated hemoglobin levels were higher in the observation group than in the control group,the difference was statistically significant(P<0.05);total cholesterol,triacylglycerol and low-density lipoprotein cholesterol levels were higher in the observation group than in the control group,and high-density lipoprotein cholesterol levels were lower than in the control group,the difference was statistically significant(P<0.05).The sensitivity,specificity,crude agreement,positive predictive value and negative predictive value of diagnostic test 1 were higher than those of diagnostic test 2,the difference was statistically signifi cant(P<0.05).Conclusion The differences in glucose and lipid tests in type 2 diabetic patients compared with healthy physical examiners were significant and beneficial in guiding clinical treatment activities.The sensitivity,specificity,crude agreement and predictive value of diagnostic test 1 were all at a higher level and higher than that of diagnostic test 2,and the clinical diagnostic value of test 1 was higher.
