摘要
2型糖尿病(type 2 diabetes mellitus,T2DM)是临床上常见的慢性代谢性疾病,以慢性高血糖为特点,其中,胰岛素抵抗和胰岛β细胞功能障碍是2型糖尿病最基本的病理生理特征。肺结核(pulmonary tuberculosis,PTB)是由结核分枝杆菌引起的一种慢性传染性疾病,会对糖尿病患者胰岛β细胞功能产生不利影响,导致疾病进展或恶化。目前肺结核对2型糖尿病患者胰岛β细胞功能影响的研究仍较少。本文通过高血糖、慢性炎症、应激反应和免疫反应4个方面,阐述肺结核对2型糖尿病患者胰岛β细胞功能影响的相关机制,旨在为及时有效控制血糖、早期保护胰岛β细胞功能、延缓肺结核进展提供理论依据和参考。
Type 2 diabetes mellitus(T2DM)is a clinically common chronic metabolic disease characterized by chronic hyperglycemia,in which insulin resistance and isletβ-cell dysfunction are the most fundamental pathophysiological features of type 2 diabetes mellitus.Pulmonary tuberculosis(PTB),a chronic infectious disease caused by Mycobacterium tuberculosis,adversely affects isletβ-cell function in diabetic patients,leading to disease progression or worsening.There are still relatively few studies on the effects of tuberculosis on isletβ-cell function in type 2 diabetic patients.In this paper,we describe the mechanisms related to the effects of tuberculosis on pancreatic isletβ-cell function in type 2 diabetic patients through four aspects:hyperglycemia,chronic inflammation,stress response and immune response,aiming to provide theoretical basis and reference for timely and effective glycemic control,early protection of pancreatic isletβ-cell function and delaying the progression of tuberculosis.
