摘要
目的探讨CysC、HbA1c、Cr、BUN水平在糖尿病肾病早期诊断中的作用。方法选取2020年1月—2022年12月于宜兴市第二人民医院就诊的早期糖尿病肾病患者104例作为糖尿病肾病组,另选取104例单纯糖尿病患者作为糖尿病组、104例健康人群作为健康组。分别比较3组胱抑素C(CysC)水平、糖化血红蛋白(HbA1c)水平、肌酐(Cr)水平和血尿素氮(BUN)等生化指标水平,及各指标对糖尿病肾病的诊断价值。结果糖尿病肾病组CysC、HbA1c、Cr、BUN水平高于糖尿病组、健康组,差异有统计学意义(P<0.05)。CysC、HbA1c、Cr、BUN对糖尿病肾病的灵敏度、特异性、准确度均较高(P<0.001),绘制CysC、HbA1c、Cr、BUN诊断糖尿病肾病的ROC曲线,显示AUC分别0.78、0.91、0.86、0.78。结论对生化指标CysC、HbA1c、Cr、BUN进行检测,可在早期确诊糖尿病肾病,上述生化指标可更好地反应患者病情,对临床治疗和预后具有重要意义,检测手段值得推广。
Objective To explore the role of CysC,HbA1c,Cr,BUN levels in the early diagnosis of diabetes nephropathy.Methods 104 patients with early diabetes nephropathy admitted to the Second People's Hospital of Yixing City from January 2020 to December 2022 were selected as the diabetes nephropathy group,104 patients with simple diabetes were selected as the diabetes group,and 104 healthy people were selected as the health group.Cystatin C level,glycated hemoglobin level,creatinine(Cr)level and blood urea nitrogen(BUN)level were compared among the three groups,and the diagnostic value of each index for diabetes nephropathy.Results The levels of CysC,HbA1c,Cr and BUN in the diabetes nephropathy group were higher than those in the diabetes group and the healthy group,the difference was statistically significant(P<0.05).CysC,HbA1c,Cr,and BUN had high sensitivity,specificity,and accuracy for diabetes nephropathy(P<0.001).Draw ROC of CysC,HbA1c,Cr,BUN for diagnosis of diabetes nephropathy,and show AUC 0.78,0.91,0.86,0.78 respectively.Conclusion The detection of biochemical indexes CysC,HbA1c,Cr and BUN can confirm the diagnosis of diabetic nephropathy patients at an early stage,and the above biochemical indexes can better respond to the patient's condition,which has important significance for clinical treatment and prognosis,and the detection means is worth promoting.
