运动疗法对青少年特发性脊柱侧弯干预效果的网状Meta分析 被引量:2

Mesh Meta-analysis of Intervention Effect of Exercise Therapy on Adolescent Idiopathic Scoliosis

摘要 目的评价不同运动疗法对青少年特发性脊柱侧弯的干预效果。方法检索中国知网(CNKI)、万方数据库、维普数据库、PubMed、Web of Science等数据库,筛选符合纳入标准的文献,采用Cochrane手册推荐的RCT偏倚风险评估工具对文献进行质量评价,运用Stata 16.0软件进行网状Meta分析。结果最终纳入17项研究,共有1059例受试者,共7种运动疗法。Cobb角方面,Schroth疗法(WMD=-4.49,95%CI[-7.44,-1.54],p<0.05)、BSPTS(WMD=-5.90,95%CI[-11.52,-0.28],p<0.05)、SEAS(WMD=-3.62,95%CI[-6.67,-0.57],p<0.05)的改善效果显著;ATR方面,Schroth疗法(WMD=-3.58,95%CI[-6.67,-0.49],p<0.05)、SEAS(WMD=-2.77,95%CI[-5.42,-0.13],p<0.05)的改善效果显著。SUCRA概率排序结果显示,BSPTS(SUCRA=83.1)对于改善Cobb角指标起最显著作用;Schroth疗法(SUCRA=80.3)对于改善ATR指标起最显著作用。结论多种运动疗法能够改善AIS,尤其以BSPTS、Schroth疗法的效果最为显著。 Objective Assessing the intervention efficacy of disparate therapeutic exercise methods on juvenile idiopathic scoliosis.Method Comprehensive database exploration was carried out utilizing CNKI,Wanfang,VIP Database,PubMed and Web of Science.The selection of documents was governed by predetermined inclusion criteria.Quality appraisal of the selected studies was completed using the Risk of Bias assessment tool as recommended by the Cochrane Collaboration for randomized controlled trials.Network Meta-analysis was executed utilizing Stata 16.0 statistical software.Results A total of 17 studies were eventually included,with 1,059 participants and seven types of physical therapy.In terms of the Cobb angle,Schroth therapy(WMD=-4.49,95%CI[-7.44,-1.54],p<0.05),BSPTS(WMD=-5.90,95%CI[-11.52,-0.28],p<0.05),and SEAS(WMD=-3.62,95%CI[-6.67,-0.57],p<0.05)had significant improvement effects.In terms of the ATR,Schroth therapy(WMD=-3.58,95%CI[-6.67,-0.49],p<0.05),and SEAS(WMD=-2.77,95%CI[-5.42,-0.13],p<0.05)showed significantly beneficial effects.The SUCRA(Surface Under the Cumulative Ranking)probability ranking results show that BSPTS(SUCRA=83.1)played the most significant role in improving the Cobb angle indicator.Schroth therapy(SUCRA=80.3)played the most significant role in improving the ATR indicator.Conclusion Various therapeutic exercises can improve AIS(adolescent idiopathic scoliosis),with BSPTS and Schroth therapy particularly showing the most notable effects.
