摘要
目的探讨经皮球囊扩张椎体后凸成形术(PKP)治疗多发性骨髓瘤(MM)导致的椎体骨折的疗效。方法选取MM导致椎体骨折患者35例(共49个椎体),采用PKP治疗,对比手术前后视觉模拟评分(VAS)、Barthel指数、脊柱功能障碍指数(ODI)评分及椎体高度变化。结果35例(共49个椎体)患者手术均成功,其中患者术后生命体征良好,随访期间共2例(3个椎体)患者死亡,死亡原因分别为1例骨髓功能衰竭,1例感染。与术前相比,术后1 d、3个月、6个月VAS评分、ODI评分降低,Barthel指数升高(P<0.05);与术前相比,术后6个月椎体前缘高度、中份高度、后缘高度均较高(P<0.05)。结论PKP治疗MM导致椎体骨折患者可显著缓解疼痛,提升日常生活活动能力,促进脊柱功能恢复,改善椎体畸形,临床效果显著。
Objective To investigate the efficacy of percutaneous kyphoplasty(PKP)with balloon dilatation in the treatment of vertebral fractures caused by multiple myeloma(MM).Methods 35 patients(49 vertebrae in total)with vertebral fracture caused by MM were selected and treated with PKP.The visual analog score(VAS),Barthel index,spinal dysfunction index(ODI)and vertebral height before and after operation were compared.Results 35 patients(49 vertebrae in total)were successfully operated,of which the postoperative vital signs were good.During the follow-up period,2 patients(3 vertebrae)died.The causes of death were 1 case of bone marrow failure and 1 case of infection.Compared with that before operation,VAS score and ODI score decreased at 1 day,3 months and 6 months after operation,while Barthel index increased(P<0.05);Compared with that before operation,the anterior,middle and posterior heights of the vertebral body were higher 6 months after operation(P<0.05).Conclusion PKP can significantly relieve pain,improve activities of daily living,promote the recovery of spinal function,and improve vertebral deformity in patients with MM induced vertebral fracture.
