摘要
目的比较老年精神分裂症患者采用氯氮平与喹硫平治疗对精神行为症状、认知功能、血清泌乳素(PRL)水平及体重指数(BMI)的影响。方法选取2020年5月至2022年3月新乡医学院第二附属医院收治的共计76例老年精神分裂症患者,以随机数字表法分成研究组(38例)与对照组(38例),对照组接受氯氮平治疗,研究组接受喹硫平治疗,比较两组精神行为症状、认知功能、血清泌乳素水平及BMI。结果治疗后两组阳性与阴性症状量表(PANSS)评分均降低,研究组评分较对照组更低(P<0.05);两组蒙特利尔认知评估量表(MoCA)、简易精神状态检查(MMSE)评分均提高,研究组评分较对照组更高(P<0.05);两组PRL水平均提高,但研究组水平较对照组低(P<0.05);对照组治疗后BMI提高(P<0.05),但研究组治疗前后BMI差异无统计学意义(P>0.05),治疗后研究组BMI较对照组低(P<0.05)。结论相较于氯氮平,喹硫平应用于老年精神分裂症患者治疗中,能够更好地改善精神行为症状与认知功能,降低对PRL水平及BMI的影响。
Objective To compare the effects of clozapine and quetiapine on psychosocial symptoms,cognitive function,serum prolactin(PRL)level and body mass index(BMI)in elderly schizophrenics.Methods A total of 76 elderly schizophrenics who were admitted in the Second Affiliated Hospital of Xinxiang Medical College from May 2020 to March 2022 were selected,and they were divided into the study group(38 cases)and the control group(38 cases)by random number table method.The control group was treated with clozapine,and the study group was treated with quetiapine.The psychobehavioral symptoms,cognitive function,serum prolactin level and BMI of the two groups were compared.Results After treatment,the scores of positive and negative symptom scale(PANSS)in both groups were decreased,and the score of the study group was lower than that of the control group(P<0.05).The scores of Montreal cognitive assessment scale(MoCA)and mini-mental state examination(MMSE)in both groups were improved,and the scores in the study group were higher than those in the control group(P<0.05).The PRL levels in both groups were increased,but the level in the study group was lower than that in the control group(P<0.05).The BMI of the control group were increased(P<0.05),but there was no statistically significant difference in the BMI of the study group before and after treatment(P>0.05),and the BMI of the study group after treatment was lower than that of the control group(P<0.05).Conclusion Compared with clozapine,quetiapine used in the treatment of elderly schizophrenia can better improve the psychobehavioral symptoms and cognitive function,and reduce the impact on PRL level and BMI.
