摘要
目的 观察Peplau人际关系理论联合“5A”护理模式在结直肠癌患者护理中的应用效果,以及对其术后并发症和康复效果的影响。方法 以2021年7月至2022年7月厦门大学附属中山医院收治的96例结直肠癌患者为研究对象,采用抽签法分为对照组和观察组,每组48例。对照组给予基础康复护理,观察组给予Peplau人际关系理论加“5A”护理。比较2组患者的康复指标、术后并发症发生情况、营养指标和生活质量。结果 观察组的首次进食时间、肠鸣音恢复时间和首次排便时间均早于对照组,住院时间短于对照组,并发症总发生率低于对照组,差异均有统计学意义(P均<0.05)。护理1个月后,2组的血清白蛋白(ALB)、前白蛋白(PAB)和转铁蛋白(TRF)水平均高于护理前;观察组的ALB、PAB和TRF水平均高于对照组,差异均有统计学意义(P均<0.05)。护理1个月后,2组的生活质量调查问卷(EORTC QLQ-C30)评分均高于护理前,胃肠道生活质量指数(GIQLI)评分均低于护理前;观察组的EORTC QLQ-C30评分高于对照组,GIQLI评分低于对照组,差异均有统计学意义(P均<0.05)。结论 Peplau人际关系理论加“5A”护理在结直肠癌患者的临床护理中具有较高的应用价值,能促进患者术后康复,改善其营养状况和生活质量,并减少并发症的发生。
Objective To observe the application effect of Peplaus theory of interpersonal relations combined with 5A nursing model in the nursing of colorectal cancer patients and its influence on the postoperative complications and rehabilitation.Methods A total of 96 colorectal cancer patients admitted to Zhongshan Hospital Xiamen University from July 2021 to July 2022 were enrolled and divided into a control group and an observation group by drawing lots,with 48 cases in each group.The control group was given basic rehabilitation nursing,and the observation group was given Peplaus theory of interpersonal relations combined with 5A nursing.The rehabilitation indicators,postoperative complications,nutritional indicators,and quality of life of the two groups were compared.Results The first feeding time,bowel sound recovery time,first defecation time,and length of hospital stay of the observation group were shorter than those of the control group,and the total incidence of complications was lower than that of the control group.The differences were statistically significant(all P<0.05).After 1 month of nursing,the serum albumin(ALB),prealbumin(PAB),and transferrin(TRF)levels in the two groups were higher than those before nursing.The ALB,PAB,and TRF levels in the observation group were higher than those in the control group.The differences were statistically significant(all P<0.05).After 1 month of nursing,the Quality of Life Questionnaire Core 30(EORTC QLQ-C30)scores of the two groups were higher than those before nursing,and the Gastrointestinal Quality of Life Index(GIQLI)scores were lower than those before nursing.The EORTC QLQ-C30 score of the observation group was higher than that of the control group,and the GIQLI score was lower than that of the control group,with statistical differences(all P<0.05).Conclusion Peplaus theory of interpersonal relations combined with 5A nursing has a high application value in the clinical nursing of colorectal cancer patients,which can promote their postoperative rehabilitation,improve their nutritional status and quality of life,and reduce the complications.
