摘要
目的运用数据挖掘技术分析针灸治疗梨状肌综合征(PS)的选穴规律。方法检索中国知识资源总库(CNKI)、万方数据知识服务平台(万方数据)、维普中文期刊服务平台(维普网)、中国生物医学文献数据库(CBM)、PubMed、Embase、Cochrane Library建库至2022年10月1日收录的针灸治疗PS的临床研究文献,利用Excel2019建立处方用穴数据库,采用SPSS Modeler 18.0、SPSS Statistics 26.0软件对腧穴使用频数、归经、部位、特定穴进行统计,并进行关联规则分析、聚类分析,探究针灸治疗PS的选穴特点及规律。结果共纳入162篇文献,包含80个腧穴,总频次为1054次。高频腧穴为环跳、阳陵泉、委中等,常用经脉为足太阳膀胱经和足少阳胆经,腧穴多位于下肢部和臀部,以五输穴和下合穴居多。核心穴组为“环跳-委中-阳陵泉”,并得到20组关联规则和4个有效聚类。结论针灸治疗PS多选取环跳、阳陵泉、委中等膀胱经与胆经腧穴,注重下肢部和臀部腧穴的运用,选穴遵循行气活血、舒筋活络治疗法则,并重视经脉循行。
Objective To analyze the acupoint selection law in acupuncture and moxibustion in treating piriformis syndrome(PS)by using data mining technology.Methods The clinical research literature on acupuncture and moxibustion treatment for PS was retrieved from CNKI,Wanfang Data,VIP,CBM,PubMed,Embase,Cochrane Library from the establishment of the databases to October 1,2022.The acupoint prescription database was established using Excel 2019.The frequency of acupoints,meridian analysis,location,and specific acupoints were analyzed using SPSS Modeler 18.0 and SPSS Statistics 26.0.Association rule analysis and clustering analysis were performed to explore the characteristics and law of acupoint selection for acupuncture and moxibustion treatment of PS.Results Totally 162 articles were included,including 80 acupoints,with a total frequency of 1054 times.The high-frequency acupoints were Huantiao(GB30),Yanglingquan(GB34),Weizhong(BL20),etc.The commonly used meridians were bladder meridian and gallbladder meridian.The involved acupoints were mostly located in the lower limbs and buttocks,and most of them were Wushu and Xiahe acupoints.The core acupoints group was“Huantiao(GB30)-Weizhong(BL20)-Yanglingquan(GB34)”,and 20 association rules and 4 effective cluster groups were obtained.Conclusion Acupuncture and moxibustion treatment for PS mostly selects Huantiao(GB30),Yanglingquan(GB34),Weizhong(BL20),bladder meridian and gallbladder meridian,pays attention to the use of acupoints on the lower limbs and buttocks.The acupoint selection follows the therapeutic principle of promoting qi and blood circulation,relaxing muscles and activating collaterals,and attaches importance to the circulation of meridians.
