摘要
目的探讨健脾消萎汤治疗老年慢性萎缩性胃炎(senile chronic atrophic gastritis,SCAG)肝胃郁热型患者的临床疗效。方法回顾性分析2017年6月—2020年6月安徽中医药大学附属第一医院老年病中心脾胃病科60例老年肝胃郁热型SCAG患者的临床数据,根据不同治疗方法将60例SCAG患者分为观察组(接受西医对症处理联合健脾消萎汤治疗,n=30)和对照组(仅接受西医对症治疗,n=30)。观察并比较2组临床疗效、中医证候评分、胃镜下黏膜病变评分(gastroscopy index,GI)、胃黏膜组织病理学变化评分(gastric mucosal lesion index,PI)以及血清胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ和胃泌素-17水平的变化。结果观察组SCAG患者治疗后总有效率明显高于对照组(90%vs 66.67%)(P<0.05)。治疗后,观察组的胃痛、嘈杂、心烦易怒和反酸症状明显改善(P<0.05)。治疗后,观察组GI和PI明显低于对照组(P<0.05)。治疗后,观察组血清PG-Ⅰ、PG-Ⅰ/PG-Ⅱ和G-17水平明显高于对照组(P<0.05)。结论健脾消萎汤联合西药治疗老年肝胃郁热证型SCAG患者,可有效减轻患者的临床症状,改善组织学病变和胃功能,提高患者的生活质量,具有一定的临床应用价值。
Objective To explore the clinical efficacy of Jianpi Xiaowei decoction(JXD)in the treatment of elderly patients with chronic atrophic gastritis(SCAG)of liver and stomach gloomy heat type.Methods The clinical data of 60 elderly patients with SCAG of liver and stomach gloomy heat type treated in Department of Gastroenterology,Geriatric Disease Center,First Affiliated Hospital of Anhui University of Traditional Chinese Medicine(TCM)from June 2017 to June 2020 were retrospectively analyzed.According to different treatment methods,they were divided into observation group(receiving Western medicine symptomatic treatment combined with JXD,n=30)and control group(receiving Western medicine symptomatic treatment only,n=30).The clinical efficacy,TCM syndrome score,mucosal lesion score under gastroscopy(GI),gastric mucosal histopathological changes score(PI)and the levels of serum pepsinogenⅠ,pepsinogenⅡand gastrin-17 were observed and compared between the two groups.Results The total effective rate of the observation group was significantly higher than that of the control group(90%vs 66.67%)(P<0.05).After treatment,the symptoms of stomachache,noise,irritability and acid reflux were significantly improved in the observation group(P<0.05).After treatment,the GI and PI of the observation group were significantly lower than those of the control group(P<0.05),and the levels of serum PG-Ⅰ,PG-Ⅰ/PG-Ⅱ,and G-17 of the observation group were significantly higher than those of the control group(P<0.05).Conclusion The combination of JXD and Western medicine in the treatment of elderly patients with SCAG of liver and stomach gloomy heat type can effectively alleviate their clinical symptoms,improve histopathological changes and gastric function,and improve their quality of life.It has certain clinical application value.
