摘要
目的探讨胸部CT灌注成像对肺癌淋巴结转移的预测价值。方法收集120例肺癌患者的临床资料,有淋巴结转移70例,无淋巴结转移50例。对所有肺癌患者进行CT平扫和多层螺旋CT灌注扫描。比较有淋巴结转移和无淋巴结转移患者的胸部CT灌注成像参数,并分析胸部CT灌注成像参数中的表面通透性和灌注值预测淋巴结转移的价值。结果有淋巴结转移患者肿瘤直径大于无淋巴结转移患者,周围型肺癌和鳞状上皮细胞癌比例均低于无淋巴结转移患者,差异均有统计学意义(P<0.05)。有淋巴结转移患者表面通透性和灌注值均明显高于无淋巴结转移患者,差异均有统计学意义(P<0.01)。表面通透性预测肺癌淋巴结转移的灵敏度、特异度、准确度分别为77.80%、84.00%、75.00%,曲线下面积为0.796;灌注值预测肺癌淋巴结转移的灵敏度、特异度、准确度分别为60.00%、70.00%、78.00%,曲线下面积为0.692。结论胸部CT灌注成像可有效预测肺癌淋巴结转移情况,具有临床指导意义。
Objective To explore the predictive value of chest CT perfusion imaging in lymph node metastasis of lung cancer.Method The clinical data of 120 patients with lung cancer were collected,including 70 cases with lymph node metastasis and 50 cases without lymph node metastasis.All patients were examined with plain CT scan and multislice spiral CT perfusion scan.The chest CT perfusion imaging parameters of patients with lymph node metastasis and without lymph node metastasis were compared,and the value of surface permeability and perfusion value of chest CT perfusion imaging parameters in predicting lymph node metastasis was analyzed.Result The tumor diameter in the patients with lymph node metastasis was larger than that in the patients without lymph node metastasis,and the proportion of peripheral lung cancer and squamous cell carcinoma were lower than those in the patients without lymph node metastasis(P<0.05).The surface permeability and perfusion value of patients with lymph node metastasis were higher than those without lymph node metastasis(P<0.01).The sensitivity,specificity and accuracy of surface permeability in predicting lymph node metastasis were 77.80%,84.00%,75.00%,and the area under the curve was 0.796.The sensitivity,specificity and accuracy of perfusion value in predicting lymph node metastasis were 60.00%,70.00%,78.00%,and the area under the curve was 0.692.Conclusion Chest CT perfusion imaging can effectively predict the lymph node metastasis of lung cancer,which has clinical significance.
