摘要
目的:观察超声乳化联合囊袋张力环(CTR)及人工晶体植入术(ICL)在晶体悬韧带异常白内障患者中的临床应用效果。方法:收集医院2019年1月-2021年6月收治的79例复杂白内障合并悬韧带异常患者资料,根据患者不同手术方案划分为对照组(41例,超声乳化联合ICL)、研究组(38例,超声乳化联合CTR及ICL);对比两组术前、术后3个月、术后6个月、术后12个月视力、眼压、中央前房深度、人工晶体稳定性及黄斑变化检查结果。结果:两组患者术前视力、眼压、中央前房深度对比差异无统计学意义(P>0.05);研究组术后3个月、6个月、12个月视力值、中央前房深度较对照组高,眼压均较对照组低,差异有统计学意义(P<0.05);研究组术后3个月、6个月、12个月人工晶体偏心量、人工晶体倾斜度对比差异无统计学意义(P>0.05);对照组术后6个月、12个月人工晶体偏心量、人工晶体倾斜度均较术后3个月升高,且各时点人工晶体偏心量、人工晶体倾斜度均较研究组高,差异有统计学意义(P<0.05);两组术后黄斑水肿、厚度增加发生率对比差异无统计学意义(P>0.05)。结论:晶体悬韧带异常白内障患者采用超声乳化联合CTR及ICL治疗可改善视力,降低眼压,提高中央前房深度,且人工晶体稳定性更好。
Objective:To observe the clinical application effect of phacoemulsification combined with Capsular Tension Ring(CTR)and intraocular lens implantation(ICL)in cataract patients with abnormal zonule.Methods:The data of 79 patients with complex cataract complicated with zonular abnormal cataract admitted to our hospital from January 2019 to June 2021 were collected.According to different surgical schemes,the patients were divided into control group(41 cases,phacoemulsification combined with ICL)and study group(38 cases,phacoemulsification combined with CTR and ICL).The visual acuity,intraocular pressure,central anterior chamber depth,intraocular lens stability and macular changes were compared between the two groups before operation,3 months,6 months and 12 months after operation.Results:There was no significant difference in preoperative visual acuity,intraocular pressure and central anterior chamber depth between the two groups(P>0.05).The visual acuity and central anterior chamber depth of the study group were higher than those of the control group at 3 months,6 months and 12 months after operation,and the intraocular pressure was lower than that of the control group(P<0.05).The eccentricity and inclination of the intraocular lens in the control group at 6 months and 12 months after operation were higher than those at 3 months after operation,and the eccentricity and inclination of the intraocular lens at each time point were higher than those in the study group(P<0.05).There was no significant difference in the incidence of macular edema and thickness increase between the two groups(P>0.05).Conclusion:Phacoemulsification combined with CTR in cataract patients with abnormal lens suspensory ligament.
