摘要
目的探讨呼吸机依赖患者脱机尝试进程中介入渐进性抗阻力干预方案指导对脱机成功率以及焦虑情绪的影响。方法借助随机数字表法将本院ICU科室2019至2020年收治的80例呼吸机依赖患者分为对照组和研究组,每组40例,对照组脱机尝试进程中接受常规护理,研究组脱机尝试进程中加用渐进性抗阻力干预,观察2组脱机时间、一次脱机成功率、脱机成功率、并发症情况,护理前后汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)和MRC肌力评分。结果研究组脱机时间、并发症率低于对照组(P<0.05),一次脱机成功率、脱机成功率均高于对照组(P<0.05)。2组患者护理前HAMA、HAMD评分、MRC评分比较,差异无统计学意义(P>0.05),护理后研究组HAMA、HAMD评分低于对照组,MRC评分高于对照组,差异均有统计学意义(P<0.05)。结论将渐进性抗阻力干预应用到呼吸机依赖患者脱机工作中,可有效缓解其焦虑、抑郁情绪对脱机工作带来的不良影响,缩短脱机时间,提高脱机成功率,减少并发症发生的同时,改善肌力水平。
Objective To investigate the influence of progressive anti-resistance nursing program guidance on the success rate of weaning and anxiety in patients with ventilator dependence during weaning attempt.Methods A total of 80 ventilator-dependent patients admitted to ICU of our hospital from January 2019 to December 2020 were randomly allocated to control group and research group,with 40 patients in each group.During the offline trial process,Routine nursing was given to all patients,and progressive resistance training was additionally given to those of research group.The weaning time,first weaning success rate,weaning success rate,complications,Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD)and Medical Research Council(MRC)Scale scores were compared.Results Compared with the control group,the research group had significantly shorter weaning time,lower complication rate,higher first weaning success rate and weaning success rate(P<0.05).There were no significant differences in HAMA,HAMD and MRC scores before nursing between groups(P>0.05).After nursing,significantly lower HAMA and HAMD scores,and higher MRC scores were detected in research group than those in control group(P<0.05).Conclusion progressive resistance training can effectively alleviate the anxiety and depression,shorten the time of weaning,enhance the success rate of weaning,reduce the incidence of complications and improve muscle strength in ventilator-dependent patients.
