摘要
目的 探讨中药贴敷联合吸入糖皮质激素治疗儿童哮喘的疗效及对Th1/Th2细胞因子的影响。方法 选取2017年6月—2019年6月期间湖北省恩施州民族医院儿科收治的支气管哮喘患儿100例,采用随机摸球法分为对照组和联合组,每组各50例。对照组采用吸入糖皮质激素治疗,联合组采用中药贴敷联合吸入糖皮质激素治疗。治疗3个月后评价两组患者临床疗效,观察两组患者治疗前后肺功能指标[肺活量(Forced vital capacity, FVC)、1秒用力呼吸量(Forced expiratory volume in one second, FEV1)、最大呼气峰流速(Peak Expiratory Fowl rate, PEF)]及Th1/Th2细胞因子[白细胞介素-12(Interleukin-12,IL-12)、干扰素γ(Interferon-γ,IFN-γ)、IL-4(Interleukin-4,IL-4)、IL-13(Interleukin-13,IL-13)]的变化。结果 治疗后联合组总有效率92.00%(46/50)高于对照组74.00%(37/50),差异有统计学意义(P<0.05)。治疗后两组患者肺功能指标较治疗前升高,差异有统计学意义(P<0.05);且联合组肺功能指标FVC、FEV1、PEF均高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清IL-12、IFN-γ水平较治疗前升高,血清IL-4、IL-13水平较治疗前降低,差异有统计学意义(P<0.05);且联合组血清IL-12、IFN-γ水平升高程度,血清IL-4、IL-13水平降低程度均优于对照组,差异有统计学意义(P<0.05)。结论 中药敷贴联合吸入糖皮质激素对小儿哮喘临床缓解期的治疗具有积极作用,改善肺功能,纠正Th1/Th2免疫失衡。
Objective To investigate the efficacy of Chinese medicine application combined with inhaled glu-cocorticoids in the treatment of children with asthma and its effect on Th1/Th2 cytokines.Methods A total of 100 chil-dren with bronchial asthma admitted to the Department of Pediatrics of HubeiEnshi Prefecture Nationalities Hospitalfrom June 2017 to June 2019 were selected and divided into a control group and a combined group by random ball-grasping method,with 50 in each group.The control group was treated with inhaled glucocorticoids,while the combined group was treated with Chinese medicine application combined with inhaled glucocorticoids.After 3 months of treatment,the clinical efficacy of the two groups was evaluated,and the changes in lung function indexes[forced vital capacity(FVC),forced ex-piratory volume in one second(FEV1),peak expiratory fowl rate(PEF)]and Th1/Th2 cytokines[interleukin-12(IL-12),interferonγ(IFN-γ),IL-4,IL-13]of the two groups before and after treatment were observed and compared.Re-sults The total effective rate of the combined group(92.00%,46/50)was higher than that(74.00%,37/50)of the con-trol group(P<0.05).After treatment,the levels of lung function indexes(FVC,FEV1 and PEF)in both groups were in-creased(P<0.05),and the combined group was higher than the control group in the above indexes(P<0.05).Addition-ally,the levels of serum IL-12 and IFN-γin the two groups were elevated while the levels of IL-4 and IL-13 were de-creased compared with those before treatment(P<0.05),and the combined group had more increase or decrease than the control group(P<0.05).Conclusion Chinese medicine application combined with inhaled glucocorticoids has a positive effect on the treatment of children with asthma in remission period,which improves lung function and corrects Th1/Th2 im-balance.
