摘要
目的探究分析在糖尿病早期肾损伤中采用尿糖联合尿白蛋白肌酐比值检验的诊断价值。方法选取2021年1月—2022年4月期间内124例于常州市武进人民医院检查的糖尿病疑似早期肾损伤患者作为研究对象。所有检查人员均通过尿糖检验、尿白蛋白与肌酐比值检验。比较阳性组(110例)、阴性组(14例)尿糖水平、尿白蛋白肌酐比值水平。比较单一检查和联合检查在糖尿病早期肾损伤患者中的诊断效能。结果阳性组患者的尿糖、尿白蛋白肌酐比值水平均明显高于阴性组,差异有统计学意义(P<0.05)。尿糖与尿白蛋白肌酐比值联合检验的检出率为85.48%、灵敏度95.46%、特异度92.86%、准确度95.16%高于单一尿糖检测的82.26%、88.18%、64.29%、85.48%,差异有统计学意义(χ^(2)=0.477、3.872、3.394、6.643,P=0.490、0.049、0.065、0.010)。结论在糖尿病早期肾损伤临床诊断中采用尿糖联合尿白蛋白肌酐比值检验方法具有较高诊断价值,可为临床诊断提供有力数据支持,为判断是否出现早期肾损伤起到指导作用。
Objective To investigate and analyze the diagnostic value of using glycosuria combined with urinary albumin creatinine ratio test in early diabetic kidney injury.Methods From January 2021 to April 2022,one hundred and twenty-four patients with suspected early diabetic kidney injury examined at Changzhou Wujin People's Hospital were selected as the subjects All examiners were tested by glycosuria test and urinary albumin creatinine ratio test.Glycosuria level and urinary albumin creatinine ratio level were compared between positive group(one hundred and ten cases)and negative group(fourteen cases).The diagnostic efficacy of single test and combined test were compared in patients with early diabetic kidney injury.Results The glycosuria and urinary albumin creatinine ratio levels in the positive group were significantly higher than in the negative group,the difference was statistically significant(P<0.05)The detection rate 85.48%,sensitivity 95.46%,specificity 92.86%and accuracy 95.16%of the combined test for the ratio of urine glucose to urinary albumin creatinine were higher than those of the single test for urine glucose 82.26%,88.18%,64.29%and 85.48%,the difference was statistically significant(χ^(2)=0.477,3.872,3.394,6.643,P=0.490,0.049,0.065,0.010).Conclusion In the clinical diagnosis of early kidney injury in diabetes mellitus,the use of glycosuria combined with urinary albumin creatinine ratio test has high diagnostic value,which can provide strong data support for clinical diagnosis and play a guiding role in determining whether early kidney injury occurs.
