摘要
目的:探讨经尿道输尿管软镜钬激光治疗肾盏憩室脓肿的疗效及安全性。方法:回顾分析2013年6月至2022年4月为21例肾盏憩室脓肿患者经尿道行输尿软镜钬激光治疗的临床资料,患者5~78岁,平均(35±20)岁,主要症状为腰疼、尿路刺激等。病程7~60 d,平均(16.4±12.7)d,术前均行尿常规、泌尿系统彩超、CT尿路成像等检查。术后予以定期随访。结果:本组21例手术均顺利完成,术中出血量少,手术时间35~75 min,平均(51.9±9.0)min,术中无周围组织器官损伤。术后经积极抗感染治疗后体温恢复正常,无感染性休克等并发症发生;术后住院4~8 d,平均(5.7±0.8)d。术后随访3~20个月,影像学检查示15例患者憩室脓腔消失,6例憩室较前缩小,随访期间均未出现发热寒战、腰部不适等复发迹象。结论:经尿道输尿管软镜钬激光治疗肾盏憩室脓肿安全、有效且微创。
Objective:To investigate the efficacy and safety of transurethral flexible ureteroscopy with holmium laser in the treatment of renal calyceal diverticulum abscess.Methods:The clinical data of 21 patients diagnosed as renal calyceal diverticulum abscess and treated with holmium laser through flexible ureteroscopy from Jun.2013 to Apr.2022 were retrospectively analyzed.The patients were from 5 to 78 years old,the mean was(35±20)years old,main symptoms were low back pain and urinary tract irritation.The course of disease ranged from 7 to 60 d,with the mean of(16.4±12.7)d.All patients underwent routine urine examination,color ultrasound of urinary system,CT urography and other examinations before the operation,and were followed up regularly to evaluate the efficacy and safety of the procedure.Results:The operation of 21 patients in this group went smoothly,with little intraoperative blood loss.The operation time was 35 to 75 min with the mean of(51.9±9.0)min,and no damage of the surrounding tissues and organs occurred during the operation.After active anti-infection treatment,the body temperature returned to normal,and no complications such as septic shock occurred.The postoperative hospital stay was 4-8 d,the mean was(5.7±0.8)d.During the follow-up from 3 to 20 months after the operation,imaging examination showed that the abscess cavity of diverticulum disappeared in 15 patients,and the diverticulum shrunk in 6 patients.During the follow-up period,no signs of recurrence such as fever,chills and lumbar discomfort were found in all patients.Conclusions:Transurethral flexible ureteroscopy with holmium laser is a safe,effective and minimally invasive surgical method for the treatment of renal calyx diverticulum abscess.
