经鼻高流量湿化与无创呼吸机在肺心病并发Ⅱ型呼吸衰竭患者中的应用效果 被引量:20

摘要 目的探讨经鼻高流量湿化与无创呼吸机在肺心病并发Ⅱ型呼吸衰竭患者中的疗效。方法选取2020年3月至2021年6月因肺心病并发Ⅱ型呼吸衰竭就诊的80例患者作为分析对象,随机分为2组,每组4例。A组患者采用无创呼吸机辅助通气,B组患者经鼻高流量湿化氧疗,观察22组患者的血气中pH值、PaO_(2)、PaCO_(2)、血氧饱和度,同时观察患者心率、呼吸频率、血压数值变化情况。结果2组接受不同方法处置后在pH值、PaO_(2)、PaCO_(2)、血氧饱和度以及患者心率、呼吸频率、血压数值中的比较均有明显变化,2组患者经过治疗后,其呼吸节奏和心率有着大幅度下降,A组患者降低治二氧化碳分压较B组更明显,差异有统计学意义(P<0.05)而B组在改善氧分压的效果中有优势,差异有统计学意义(P<0.05)。结论经鼻高流量湿化与无创呼吸机在肺心病并发Ⅱ型呼吸衰竭患者中的疗效各有优点,在临床上可以根据患者自身体质和疾病情况选择更加适宜的治疗方法。 Objective To investigate the therapeutic efficacy of humidified high-flow nasal oxygen versus non-invasive ventilation on pulmonary heart disease combined with typeⅡrespiratory failure.Methods A total of 80 patients with pulmonary heart disease combined with typeⅡrespiratory failure were as subjects.Patients in group A(n=40)were treated with noninvasive ventilation,and patients in group B(n=40)were treated with humidified high-flow nasal oxygen.The pH value,PaO_(2),PaCO_(2),oxygen saturation,heart rate,respiratory rate,blood pressure were compared.Results After treatment,there were significant changes in pH,PaO_(2),PaCO 2,blood oxygen saturation,heart rate,respiratory rate and blood pressure between the two groups.The respiratory rate and heart rate in the two groups decreased significantly.The reduction of PaCO_(2) in group A was more obvious than that in group B(P<0.05).The improvement of PaO_(2) was significantly better in group B than that in group A(P<0.05).Conclusion Humidified high-flow nasal oxygen and noninvasive ventilation have their own therapeutic advantages on pulmonary heart disease combined with typeⅡrespiratory failure.Clinically,more appropriate treatment methods can be selected according to the individualized conditions.Through the study of the two methods,it can provide a guarantee for the clinical treatment of patients with pulmonary heart disease with typeⅡrespiratory failure.
