摘要
目的探究耳科正常人低频(0.125kHz、0.25kHz)、言语频率(0.5kHz、1kHz、2kHz、4kHz)、高频(3kHz、4kHz、6kHz、8kHz)和扩展高频(9kHz、10kHz、11.2kHz、12.5kHz、14kHz、16kHz)纯音听阈与年龄相关的发展趋势。方法分别对各年龄符合纳入标准的受试者(n=106)进行双耳纯音听阈测试。将纯音测听结果按照低频(0.125kHz、0.25kHz)、言语频率(0.5kHz、1kHz、2kHz、4kHz)、高频(3kHz、4kHz、6kHz、8kHz)、扩展高频(9kHz、10kHz、11.2kHz、12.5kHz、14kHz、16kHz)分为四个组。在以上每个频率组中分别以年龄为横坐标,纯音阈值为纵坐标绘制散点图,观察纯音阈值的发展与“拐点”出现情况。再按照年龄18-25岁、26-35岁、36-45岁、46-55岁、56-65岁、66-75岁、≥76岁分为以上7个组,以年龄组为自变量进行多元线性回归分析。结果观察散点图发现,随年龄上升各频率组纯音阈值均有增长趋势,且随着频率的增加,纯音阈值增长的“拐点”逐渐提前并趋于年轻化。通过年龄段作为自变量的多元线性回归分析,结果显示,56-65岁、66-75岁、76岁以上组是低频组纯音阈值增长的影响因素(P<0.05),46-55岁、56-65岁、66-75岁、76岁以上组是言语频率组纯音阈值增长的影响因素(P<0.05),46-55岁、56-65岁、66-75岁、76岁以上组是高频组阈值增长的影响因素(P<0.05),26-35岁、36-45岁、46-55岁、56-65岁、66-75岁、76岁以上组是扩展高频组阈值增长的影响因素(P<0.05)。结论耳科正常人纯音听阈会随年龄增长出现增长趋势,并且可以观察到“拐点”,“拐点”随纯音测听频率的增加出现提前趋势,其中低频组(0.125kHz、0.25kHz)“拐点”出现在56-65岁年龄组、言语频率组(0.5kHz、1kHz、2kHz、4kHz)“拐点”出现在46-55岁年龄组、高频(3kHz、4kHz、6kHz、8kHz)“拐点”出现在46-55岁年龄组、扩展高频(9kHz、10kHz、11.2kHz、12.5kHz、14kHz、16kHz)“拐点”出现在26-35岁年龄组。
Objective To understand the trend of age-related pure tone hearing threshold changes in normal people across frequencies including extended high frequencies.Methods Binaural pure tone hearing thresholds were mea‐sured in subjects without hearing complaints(n=106)covering low frequency(0.125 and 0.25 kHz),speech frequency(0.5,1,2 and 4 kHz),high frequency(3,4,6 and 8 kHz)and extended high frequency(9,10,11.2,12.5,14 and 16 kHz).For each frequency range,scatter plot of thresholds against age was obtained and the occurrence of"inflection point"observed.Multiple linear regression analysis was conducted with age as the independent variable.Results Scatter plots showed that pure tone thresholds increased with age for all frequency ranges,and increasingly so with the"inflection point"of pure tone threshold increase gradually shifting toward a younger age as the frequency increased.Regression analysis showed that older age(56-65,66-75 and>76 years)was a factor influencing pure tone threshold increase in the low-frequency(P<0.05),and that ages older than 46-55 years were a factor influencing pure tone threshold increase in the language frequency(P<0.05)and high-frequency(P<0.05)ranges,while all age groups were correlated to threshold increase in the extended high-frequency range(P<0.05).Conclusion Pure tone hearing thresholds in people with no otologic complaints increase as age increases,with the"inflection point"advancing with increase of frequency(56 years for low-frequencies,46 years old for speech-and high-frequencies and 26 years for extended high frequencies.
