摘要
目的探究某市康复科患者住院服务利用现状,分析康复患者的再入院特征及其影响因素。方法整理某市2019年1—12月康复科患者首次住院及1年内再入院的记录,比较不同患者群体的康复服务利用特征,采用COX比例风险回归模型研究影响康复患者再入院的危险因素。结果共纳入康复患者2466人,1年再入院率为28.4%。儿童发育障碍类病组住院频繁反复、周期长、费用高。骨关节病组住院时长较短、费用较低,再入院率相对不高。神经系统疾病各病组特征存在差异,病种是康复患者再入院的最主要决定因素。COX回归分析显示:年龄是儿童发育障碍患者、成人骨关节病患者再入院的共同影响因素,差异有统计学意义(P<0.05)。结论康复患者再入院与有效康复的理念、服务及管理不强、三级康复医疗服务体系不完善有关,据此为制定更适宜的康复患者医保政策提供建议。
Objective To investigate the current status of service utilization of rehabilitation inpatients in a city,and analyze the readmission patterns of rehabilitation patients and their influencing factors.Methods Records of index hospitalization and readmission within a year of patients in the rehabilitation department in a city from January to December 2019 were collected,and the utilization characteristics of rehabilitation services in different patient groups were compared.A COX proportional hazards regression model was used to study the risk factors affecting the readmission of rehabilitation patients.Results A total of 2466 rehabilitation patients were included,with a one-year readmission rate of 28.4%.Children with developmental disabilities had frequent and recurrent hospitalizations,long lengths of stay,and high costs.Patients with bone diseases had shorter length of stay,lower costs,and a lower readmission rate.Characteristics of neurological disorders varied by disease.Disease was the most significant determinant of readmission patterns in rehabilitation patients.COX regression analysis showed that age was a common influencing factor for readmission in children with developmental disabilities and adults with bone diseases(P<0.05).Conclusion Readmission of rehabilitated patients was related to the weak concept,service and management of effective rehabilitation and the inadequate tertiary rehabilitation healthcare delivery system.Suggestions are provided for the development of health insurance policies that are more appropriate for rehabilitation patients.
